Q -- RADIOLOGIST SOL RFQ 124-0016 DUE 080699 POC Richard Houser, Contracting Officer 608-584-6120 The Federal Correctional Institution, Oxford, Wisconsin is soliciting to establish a contract with a Radiologist to provide specialty medical services in the area of radiological medicine to inmates incarcerated at said facility. The board certified/eligible radiologist will serve as Director of Radiology Services and shall provide evaluations and interpretations of roentgenograms, and special diagnostic roetgenographic procedures to include arthrograms, barium enemas, IVPs, G.I. Series, venograms, cholecystograms, and tomography. The contractor will provide specialized radiological tests and procedures on patients on referral from the institution medical staff. The contractor will provide written and/or dictated reports on all patients referred for evaluation and will include recommendations for further diagnostic testing and/or treatment, when indicated. Contractor will provide four-half hour sessions, per week for an estimated 216 sessions for the estimated contract period of October 1, 1999 throughSeptember 30, 2000. Offerors current State of Wisconsin licensure and/or certification to be considered for award. Offerors must have liability insurance issued by a responsible carrier in order to be eligible for award. A firm fixed-price contract will be awarded on or about September 10, 1999 to the lowest qualified bidder. This acquisition is 100% set-aside for small business.
