The Food and Drug Administration, Center for Devices and Radiological Health, has a requirement for Oracle Forms 3.0 conversion to Oracle Forms 4.5 utilizing an automated conversion tool. The requirement includes license to the proposed automated software including one year maintainence, conversion of 570 forms including rewrite of 3.0 triggers to 4.5 triggers, contractor documentation of all manual changes made to render application 100% operable.
