Hansford/937-267-3994 Perform Ophthalmology Services at the Dept. of
Veterans Affairs Medical Center, Dayton OH. The contractor will be
required to examine all patients eligible for ophthalmological surgical
care at the VA Medical Center, Dayton OH for both routine and emergency
examination, and routine and emergency ophthalmologic surgical
procedures. Perform routine ophthalmological testing and examination as
well as diagnosis and treatment of all and any ophthalmological
disorders. This includes, but is not limited to cataract surgery,
eyelid surgery, corneal surgery, retinal surgery and any other
examinations and surgical procedures of a ophthalmological routine. All
vitrectomy procedures performed at an outside community hospital will
be at a negotiated fixed rate and must be pre-approved by the Chief,
Surgical Service. Contractor will be required to be associated with
Ohio State University Ophthalmology teaching program. All
ophthalmologic physicians entering the contract must meet the
credentialing and privilege requirements of this medical center. The
contract base period is for one year, with the option to extend 2
