Medical Surveillance Examinations includes but is not limited to the
professional non-personal services, laboratory/chemical services,
equipment, and supplies required to provide periodic medical
surveillance examinations and reports at five locations within the
State of Illinois for the Illinois Army National Guard. The locations
are: 1. Combined Support Maintenance Shop, North Riverside, IL; 2. Army
Aviation Support Facility #3, Peoria, IL; 3. Combined Support
Maintenance Shop, Springfield, IL; 4. Army Aviation Support Facility
#1, Decatur, IL; 5. Operations Maintenance Shop #8, Marion, IL. All
services shall be performed in a mobile lab owned and operated by the
contractor. The following services are required at each location:
Physical Exams(complete occupational health history, physical exam,
dermatological exam, anthropometric measurements, vital signs, vision
creen, complete blood count-CBC with differencial, blood chemistries-22
item panel that includes HDLs and LDLs, PSA antigen, on board
urinalysis, audiometry, and respirator usage clearance); Pulmonary
function testing; blood lead w/ zinc protoporphyrin, and One-view(PA)
chest X-ray with B-reader; audiometric testing. All procedures will be
conducted in accordance will American Board of Occupational and
Environmental Medicine, American Association of Occupational Health
Nurese, U.S. Department of Labor, Occupational Safety and Health
Administration, National Institute for OccupationalSafety and Health
Standards. A Board Certified Occupational Medicine Physician and
Certified Occupational Health Nurse shall be available at each site.
Current estimated employees requiring these services is 282 for FY99.
Medical examination reports showing test results of each employee will
be required in disk format. The ensuing contract shall be for a base
period commencing August 1, 1999 to 30 September 1999 and will include
two one-year option periods renewable at the beginning of the federal
fiscal year. The contract willbe a firm fixed price contract. The
standard industrial classification is 8099. The small business size
standard is $5 Million. The solicitation/ request for quote will be
