a combined synopsis/solicitation for commercial items prepared in
accordance with the format in Federal Acquisition Regulation (FAR)
Subpart 12.6, as supplemented with additional information included in
this notice. This announcement constitutes the only solicitation;
quotes are being requested and a written solicitation will not be
issued. Solicitation F09609-99-Q-0009 is hereby issued as a request for
quotation (RFQ). The solicitation document and incorporated provisions
and clauses are those in effect through Federal Acquisition Circular
97-11. The procurement is 100% small business set-aside and will follow
simplified acquisition procedures in FAR Part 13. The applicable
standard industrial code (SIC) for business size determination is 3829
with a size standard of 500 employees. Dobbins Air Reserve Base,
(Marietta) Georgia is soliciting quotes for the purchase, delivery,
installation and training of a breathing air components balanced system
consisting of the following commercial items manufactured by
SUB-AQUATICS, INC.: CLIN 0001: 1 each, Medical Grade Purification
System "No. 3+" E1 with P2 Securus Monitor to include; Bauer K14-12
compressor, 67,000 cu. ft. purification system; 5,000 PSI pressure;
12.0 C.F.M., which fills a 30 minute tank in less than 4 minutes;
electronic cartridge monitor; magnetic starter; heat risers; automatic
timed drain system; high temperature shut-down; low-oil shut-down;
gauge panel with 5 gauges; accumulative hour meter; electric start on
gas and diesel; safety valves, all 4 stages; automatic pressure control
switch; power-on light, on with switch; condensate reservoir; vertical
semi-cabinet (Elect. models); adjustable final relief valve, and must
be "UL approved". CLIN 0002: 1 each, Containment Style Fill Station,
totally fragmentation enclosed (Class #2); 2 bottle fill, Model M2782,
28" wide x 29" deep x 62" high to include; front loading tilt out
door; safety bar to keep door shut; engraved control panel; adjustable
regulator; door interlock switch; bleeder valves on filler hoses; "
plate steel door, and schedule 80 aluminum tubes. CLIN 0003: 1 each,
Digital Read-Out Electronic Carbon Monoxide Monitor to include; digital
display; power cord (110 or 220 VAC or 12 VDC); regulators for air
pressure reduction; alarm light; audible option switch; solid state
electronics; compressor shut-down option; field calibration kit; NEMA
4 enclosures, and all air line connections. All items must have
complete one year parts and labor warranty and must meet all current
NFPA and OSHA standards for Safe-Fill and NFPA Grade E air. Government
personnel prior to set-up and training being conducted by the
contractor shall accomplish electrical hook-ups. Prices shall be quoted
"F.O.B. destination" with all shipping charges included in the cost of
each CLIN. The provision at FAR 52.212-1, Instructions to Offerors --
Commercial Items, shall apply along with Defense FAR Supplement
(DFARS) 252.204-7004, Required Central Contractor Registration (CCR).
Evaluation of quotes will be in accordance with FAR Clause 52.212-2,
Evaluation-Commercial Items, based on price. Quotes shall include a
completed copy of FAR Clause 52.212-3, Offeror Representations and
Certifications -- Commercial Items. FAR Clause 52.212-4, Contract Terms
and Conditions -- Commercial Items, shall apply with FAR clause
52.232-33, Payment by Electronic Funds Transfer (CCR). FAR Clause
52.212-5, Contract Terms and Conditions Required to Implement Statutes
or Executive Orders -- Commercial Items, shall be incorporated into
any contract resulting from this notice. Applicable clauses cited in
52.212-5 are 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-3 and
DFARS clauses 252.212-7001 and 252.225-7001. The full text of clauses
