612-725-2189 Provide all labor and material to replace roofs on
Buildings 89 and 68 at VA Medical Center, Minneapolis, MN 55417: (A)
BASE BID: Building #89, Roof Sections #1 and #2: Contractor to provide
a cost for the installation of an EDPM roofing system, including new
insulation, over an existing built-up roof. Work included under the
base bid shall be limited to roof sections #1 and #2 as shown on
station drawing #1037-B. Work to include the removal of any existing
debris and all preparations required for the installation of the new
roofing system. The base bid shall include the removal and repair of
600 square feet of the existing insulation and built-up roof. Existing
built-up roof repair will be limited to two locations at section #2
under the base bid. Repairs to include the removal of existing built-up
roofing and insulation and preparations for the installation of the new
EDPM roofing system; (B)ADD ALTERNATE #1, Building 89, Roof Section #3:
Contractor to provide a cost to the VA Medical Center for the
installation of an EDPM roofing system, including new insulation, over
an existing built-up roof. Work included under Add Alternate #1 will
be limited to roof section #3 as shown on station Drawing #1037-B.
Contractor shall be responsible for disconnecting the existing
electrical and mechanical systems at roof section #3. Contractor shall
remove all equipment from roof section #3 for the installation of the
new EDPM roofing system. After the roofing system has been accepted by
the VA Medical Center, the Contractor shall reinstall all electrical
and mechanical systems. Electrical and mechanical systems shall be
tested by the VA Medical Center after reinstallation. The Contractor
will be responsible for restoring the electrical and mechanical systems
to their original working condition; (C)ADD ALTERNATE #2, Building 68.
Contractor to provide a cost to the VA Medical Center for the
installation of an EDPM roofing system, including new insulation, over
an existing built-up roof at Building 68. Work to include theremoval
of any existing debris and all preparations required for the
installation of the new roofing system. All electrical and mechanical
systems and equipment shall remain in place during the installation of
the new EDPM roofing system at Building 68; (D)SQUARE FOOT UNIT COST
PROPOSAL: (1) Square Foot Costs, Built-up Roof Repair -- Building 89.
Contractor to provide a square foot unit cost for additional built-up
roofing system repairs required before the installation of the new EDPM
roofing system. Square foot unit cost will be for additional areas
beyond the 600 square feet included in the base bid. The Contractor
shall provide the equipment and services for an infrared thermal scan
inspection of roof sections 1,2 and 3. The Contractor and the VA
Medical Center's representative will mark additional built-up roof
areas to be repaired. Repairs to include the removal of existing
built-up roofing and insulation and preparations for the installation
of the new EDPM roofing system. (2) Square Foot Costs, Metal Roof Deck
Repair, Building 89. Contractor to provide a square foot cost for the
repair of the existing metal roof deck. The Contractor and a
representative of the VA Medical Center shall inspect the metal decking
to determine the total square foot of metal decking to be repaired;
(3)Square Foot Costs, Built-up Roof Repair -- Building 68. Contractor
to provide a square foot unit cost for built-up roofing system repairs
required before the installation of the new EDPM roofing system.
Contractor shall provide the equipment and services for an infrared
thermal scan inspection of the Building 68 roof. Contractor and VA
Medical Center's representative will mark built-up roof areas to be
repaired. Repairs to include the removal of existing built-up roofing
and insulation and preparations for the installation of the new EDPM
roofing system; (4) Square Foot Costs, Plywood Roof Deck Repair,
Building 68. Contractor to provide a square foot cost for the repair of
the existing plywood roof deck. The Contractor and a representative of
the VA Medical Center shall inspect the plywood decking to determine
the total square foot of plywood decking to be repaired. Conduct all
work in accordance with Drawings #1037 thru 1037-B and Specification
#99-1. A site walk to see the Buildings/Roofs will be conducted on June
16, 1999 at 9:30 a.m. All interested parties to meet at VA Medical
Center Employees Entrance off 46th Avenue Gate. A Request for Proposal
is being issued in the event that discussions have to be held with
prospective Offeror(s) prior to award. Therefore, Offerors must respond
with a technical proposal as well as a pricing proposal. Offeror must
indicate past performance naming contracts of similar scope, references
with contact name and telephone number, etc. Offerors will be evaluated
on Technical, Past Performance and Price/Price Related Factors.
Contract documents -- specification and drawings -- will be available
on or before June 2, 1999 with Offers due by close of business on July
