synopsis/solicitation for commercial items prepared in accordance with
the Subpart 12.6, as supplemented with additional information included
in this notice. This announcement constitutes the only solicitation;
quotations are being requested and a written solicitation will not be
issued. The contractor shall provide two qualified dental assistants
who will be responsible for providing assistance to the dentist at the
LAAFB dental clinic. The assistants will receive and prepare patients
for dental treatment, prepare materials and equipment for use by the
dentist, assist dentist at chairside or bedside in the treatment of
patients, taking dental radiographs and maintaining records related to
appointments, examinations, treatment and supplies. The assistants may
work in general dentistry or in a specialized field of dentistry such
as prosthodontics or oral surgery. A dental assistant may perform such
basic supportive dental procedures of the following under the general
supervision of a licensed dentist: (1) Extra-oral duties or functions
specified by the supervising dentist, and (2) Operation of dental
radiographic equipment for the purpose of oral radiography if the
dental assistant has complied with the requirements of Section 1656 of
the code. A dental assistant may perform such basic supportive dental
procedures as the following under the direct supervision of a licensed
dentist when done so pursuant to the order, control and full
professional responsibility of the supervising dentist and will be
checked and approved by the supervising dentist prior to dismissal of
the patient from the office of said dentist: (1) Take impressions for
diagnostic and opposing models, (2) Apply non-aerosol and non-caustic
topical agents, (3) Remove post-extraction and periodontal dressings,
(4) Placement of elastic orthodontic separators, (5) Remove orthodontic
separators, (6) Assist in the administration of nitrous oxide analgesia
or sedation; however, a dental assistant shall not start the
administration of the gases and shall not adjust the flow of the gases
unless instructed to do so by the dentist who shall be present at the
patient's chairside at the implementation of their instructions. This
regulation shall not be constructed to prevent any person from taking
appropriate action in the event of a medical emergency, (7) Hold
anterior matrices, (8) Remove sutures, (9) Take intra-oral measurements
for orthodontic procedures, (10) Seat adjusted retainers or headgears,
including appropriate instructions, (11) Check for loose bands, (12)
Remove arch wires, (13) Remove ligature ties, (14) Apply topical
fluoride, after scaling and polishing by the supervision dentist r a
registered dental hygienist, (15) Place and remove rubber dams, and
(16) Place, wedge and remove matrices. The following clauses will be
applicable for this acquisition: 52.212-1 Instructions to

Certifications-Commercial Items, 52.212-4 Contract Terms and
Conditions-Commercial items, 52.212-5 Contract terms and Conditions
Required to Implement Statues, and 52.237-7 Indemnification and Medical
Liability Insurance. The required liability insurance for this
acquisition is $1,000,000. Include a complete copy of the provision
52.212-3 with your offer. The clauses that are applicable from 52.212-5
are 52.222-26 Equal opportunity, 52.222-35 Affirmative action for
disabled veterans and veterans of the Vietnam era, 52.222-36
Affirmative action for handicapped workers, 52.222-37 Employment
reports on disabled veterans and veterans of the Vietnam era, 52.225-3
Buy American act-supplies, 52.222-41 service contract act of 1965 as
amended, and 52.222-44 Fair labor standards act and service contract
act-Price adjustments. The selection criteria is as follows, 52-212-2,
price and past performance. The financing clause 52.232-29, Terms for
Financing of Commercial Items will be used for this request. This
request is being set-aside for a small woman owned business. The
Standard Industrial Classification Code is 8049. The two assistants
will be hired by the LAAFB dental clinic for the period of April 1,
1999 through September 30, 1999. The hours of performance are 0645-1600
hours with a lunch break from 1145-1300 hours. The contractor shall
provide and maintain uniforms for the assigned personnel. The required
uniform is medical whites (slacks and shirts) and white hospital shoes
(water resistant). Protective equipment will be provided. All
responsible sources may submit a quotation, which shall be considered
by the agency. All interested parties must be registered in the Central
Contract Registor to due business with the federal government. See wage
