W -- LEASE AND MAINTENANCE OF REPROGRAPHIC EQUIPMENT. POC Diana Huth, Purchasing Department, Fax (213) 742-4468. Transamerica Occidental Life Insurance Company (TOLIC), which currently is under contract with HCFA for Medicare Part B claims processing, is seeking reprographic equipment to run approximately 300,000 copies per month. Seventy percent of these copies are large volume jobs (over 100 copies per request). The other 30% are small volume rush jobs, which are needed immediately. (I) TECHNICAL REQUIREMENTS -- The equipment must include, at the minimum, the following: (a) storage of a minimum of 100 five digit account numbers with usage documentation, (b) copy onto 8-1/2" x 11" x 17" paper, (c) copy reduction and enlargement, (d) minimum of three paper trays for large volume jobs, (e) manual contrast control, (f) automatic document feeder, (g) internal stapler, (h) internal binder, (i) automatic 2-sided copying, (j) ability to use 20 lb. to 65 lb. paper without excessive jamming. All machines must operate with machine effectiveness rating of a minimum of 95%. (II) OTHER REQUIREMENTS: (1) The cost for 3 yr.lease, purchase option, maintenance cost, key operator training, and equipment installation must be broken out separately. Maintenance service must be available Monday through Friday 8 a.m. to 5 p.m., (2) five client references, (3) previous experience, (4) SADBUS/MBE qualifications statement. Vendors' quotations will be evaluated 70% on cost and 30% on providing and meeting the requested information in technical and other requirements, in I and II above.
