This announcement constitutes the only solicitation to be issued; quotations are being requested for this announcement; a written solicitation will not be issued. The request for Quote number is RFQ672-093-99. This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-09. Standard Industrialization Code (SIC) of 5047 is applied. This solicitation is issued on an unrestricted basis for the following equipment Brand Name or Equal: ITEM 001, (1), One Cineangiogram Projector; To include One (1) each: Catalog no. CAP-35BIII (Cineangiogram Projector Vanguard Angiogram Projector) with mobile cart, projector hood, continuos film transport system, variable film speed from single to approximately 100 frame per second, forward & reverse drive system, 400 watt halogen lamp, push button focus control & framing system, wall projection, hand held remote control, push button zoom for up to 1.6 times for precise examination, and power single phase 60Hz 115V AC; Manuals, (2) each (Operations, Service, Parts, Schematics). ITEM 002, (1) each One Video System Option CCD Camera. $____________ Bidders quotation shall include descriptive literature (including specifications) must be submitted with quotation and preparation site requirements and item (s) not included in RFQ but required for equipment to perform its intended use. Required delivery date is 45 days from date of contract award. Inspection and Acceptance shall be at destination. Bidders shall provide the following information: current FOB destination pricing, delivery time, payment terms. Cage Code, Duns/Bradstreet number and Taxpayers I.D. number (TIN). The following provisions and clauses are applicable, are hereby incorporated into the solicitation by reference, and will be provided by the Contracting Officer in full text upon request: FAR 52.212-1, Instructions to Offerors Commercial Items applies. Addendum clause 52.215-18, Facsimile Proposals also applies. Offerors are required to complete and include a copy of the following provisions with proposals: FAR 52.212-3, Offeror Representation and Certification Commercial items, FAR 52.212-4, Contract Terms and Conditions Commercial Items, FAR 52.212-5, addendum New Material, FAR 52.212-1, Paragraph (b) (5): Offerors shall provide an express warranty which at a minimum shall be the same warranty terms, including offers of extended warranties offered to the general public.
