317-510-4702 This is a combined synopsis/solicitation for commercial
items prepared in accordance with the format in Subpart 12.6 as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; quotations are being
requested and a written solicitation will not be issued as a request
for quotation and incorporates the provisions and clauses in effect
through Federal Acquisitioin Circular 97-09. This solicitation is
set-aside for small business. The SIC Code for this solicitation is
7991 with a size standard of 500 employees. The following item is
required: Line item 0001: Purchase of 115 Memberships, as per the
following Statement of Work. The period of performance covers a period
of 3 years (base year, 2/1/99 -- 1/31/00; option period 1, 2/1/00 --
1/31/01; option period 2, 2/1/01 -- 1/31/02). Statement of Work (SOW).
Part 1 General. Access to full service physical fitness facilities for
military and civilian personnel assigned to and working at the Defense
Finance and Accounting Service-Orlando (DFAS-OR), Orlando, Florida.
The government anticipates (a) Purchase of 115 memberships for DFAS-OR
military personnel; (b) Availability of corporate membership for
DFAS-OR civilian personnel; (c) Memberships waive initial joining fee
and discount 30% from the standard open market membership fee. 1.2
Itemized Tasking. 1.2.1. Access to physical fitness facilities,
qualified instructors, formal health programs and services. 1.2.2.
Dedicated access to gymnasium floor for 100 people from 6:40AM until
7:30AM Monday, Wednesday and Friday for military/group physical fitness
training. 1.2.3. Access to the physical fitness facility will be
through individual membership cards issued to the individual by the
fitness facility personnel. Membership names will be coordinated
between Contractor and DFAS-OR. Point of contact will be given in the
award. 1.2.4 The facility is not to exceed 3 miles from the DFAS-OR,
2500 Leahy Avenue, Orlando, Florida, to reduce the amount of travel
time for military members between the fitness facility and the work
location. The facility must be easily accessible by employees before
and after duty hours and during the normal business day for those
individuals who opt to use the facility during flex hours, i.e. lunch
hour. 1.2.5. Priviledges of a regular membership to include unlimited
access to any of the contractors facility locations. 1.2.6. Services to
include, but not be limited to, use of an indoor gymnasium, community
center or recreation center complex with indoor and outdoor swimming
pools, cardiovascular equipment (minimum of 6 Stair Masters),
stationary (minimum of 24 Nautilus type machines) and free weights (to
include dumbbells, bench press, etc.), workout room, showers and
secured locker facilities and access to a 2 mile running track or
trail. Facility to be available for use from 5:30AM until 9:00PM Monday
through Friday, and on weekends. 1.2.7 Certified physical fitness
instructions, health, wellness and educational programs. Screeningto
ensure that the civilian employees enrolled in the DFAS program have
completed the fitness assessment prior to issuing a corporate
membership card. Direct billing to the civilian employee for their
corporate membership costs. DFAS-OR will not make reimbursements for
civilian employees or family memberships. Any screening necessary for
civilian and family memberships will be conducted by the contractor.
Optional family memberships and a variety of family oriented activities
are available at a discount of 30% from the standard open market
membership fee. 1.2.8. Invoice for line item 0001 is to be mailed to
DFAS-Columbus Center, ATTN: DFAS-CO-LC, P.O. Box 369016, Columbus, Ohio
43236-9016. Invoice must include purchase order number. The DFAS-OR
retains the right to terminate individual military memberships when
members are transferred from DFAS-OR or are separated from the military
service and reassign those memberships to new incoming military
members. DFAS-OR also retains the right to reduce the total number of
military memberships if downsizing occurs. Part 2 Applicable
Regulations and Documents. The following references pertain to the
physical fitness requirements and guidelines: (1) DOD Directive
1010.10; (2) Health Promotion and the American College of Sports
Medicine Guidelines (ACSM) for Exercise Testing and Prescription; (3)
AR 215-1, Moral, Welfare and Recreation Activities; and (4) AR 600-63,
Army Health Promotion. Point of contact for obtaining a copy of the

of SOW. The following FAR provisions and clauses apply to this
acquisition: 52.212-1, 52.212-2, 52.212-4 and 52.212-5. The following
evaluation criteria shall be used as addenda to FAR 52.212-2(a): (1)
Technical capability of the item offered to meet the Government
requirement; (2) price; and (3) past performance. Contractors are to
provide 3 references (name, address, phone number). The following
subparagraphs of FAR 52.212-5(b), and DFARS addenda thereto, apply:
(1), (3), (6), (7), (8) and (9); 252.225-7001 and 252.225-7036. A
completed copy of FAR 52.212-3 and the following DFARS addenda, must be
included with each quote: 252.225-7000 and 252.225-7035. Quotes are due
