The Bureau of Prisons is requesting quotations from responsible contractors with the ability to provide medical laboratory and pathology supplies and services from a certified laboratory. Services will be provided to the Federal Correctional Institution (FCI), at 27072 Ballston Road, Sheridan, Oregon 97378. The Government anticipates the award of a fixed price, requirements type contract to the entity whose offer is evaluated as being most advantageous, price and other factors considered. The anticipated period of contract performance will be Base Year, i.e., Date of Award through twelve months from award date; to be followed by four (4), one-year option periods. The anticipated date a Request for Quote (RFQ) package will be available is 12/15/98. Price quotations shall be submitted per month and will include annual totals. The RFQ package will include monthly estimates for each test identified as a requirement. The Contractor must be available to provide services as needed 365 days per year, seven days a week. Services will normally be required Monday through Friday from 8:00 a.m. to 3:30 p.m., local time (Federal holidays excluded). Price quotations must be accompanied by a detailed description of the Contractor's qualifications and experience, and must address all requirements of the aforementioned RFQ package. The anticipated date quotations will be due is 1/15/99. Requests for the RFQ package must be in writing and should be submitted before the anticipated 12/15/98 RFQ issuance date.
