The Albuquerque Area Indian Health Service will be soliciting proposals from prospective health providers for General Surgical Services for inpatient and outpatient patients served by the Acoma-Canoncito-Laguna Indian Hospital, San Fidel, New Mexico. The acquisition is set aside as a 100% Small Business Set-Aside. Contractor must possess a license to practice medicine in any one of the 50 states, show evidence that he/she is currently active in surgical work, must be able to meet the required deliverables/schedule, provide three letters of reference to include one from Chief of Staff where the provider is member of the medical staff and one from another practicing surgeon, contractor must have proof of surgical residency, curriculum vitae must show that the work reocrd includes a minimum of two years experience in rural community or services provided to a minority population, with consistent dates. Gaps in dates of employment/service must be explained. Contractor to provide proof of medical malpracticeinsurance at time of proposal submission. All proposals received will be evaluated and considered by the Government. Proposals will be evaluated based on numerically weighted technical evaluation factors. Evaluation of Technical Proposals and Cost/Price Proposals are of approximately equal value. In the case of a tie of technical proposal ranking, cost/price factors will determine the award. A single, non-personal service contract will be awarded for a base year and four, one-year options for renewal. Project date of contract award is early January 1999.
