The Request for Proposal number is 575-477-99. This solicitation incorporates provisions and clauses in effect through FAC 97-02. Standard Industrialization Code (SIC) of 8071 is applied. This solicitation is issued subject to the availability of government funds at time of award. This solicitation is for the following service: Contractor to provide emergent xray services for patients from the VA Community Clinic in Montrose, Colorado. All responsible sources located in the Montrose, CO area may submit a proposal which shall be considered by the agency. Services must be provided by a local source due to the response time necessary. The following provisions and clauses are applicable and are hereby incorporated into the solicitation by reference and will be provided by the contracting officer in full text upon request: FAR 52.212-1, Instruction to Offerors Commercial Item applies. Addendum clauses FAR 52.215-18 Facsimile Proposals also applies.
