Services are to be provided to the Albuquerque Area Indian Health Service and its surrounding Service Units. Contractor to provide 64 hours per month for 50 weeks. Contract provider must have experience/knowledge to provide leadership in Public Health Dentistry to the Area Director and Chief Medical Officer, providing advises on personnel, material, budgetary, and quality care requirements. Must be familiar with the Federal Civil Service and U.S. Public Health Service Commissioned Corps personnel systems. Must be licensed to practice dentistry in any one of the 50 States within the United States and possess a valid and current driver's license. Must have DDS or DMD, General Practice residency or 10 years experience in general dentistry and a Masters degree in Public Health. This acquisition is released as a total small business set aside. The contract will be awarded for a base year plus four, one-year options for renewal, total contract not to exceed five years. The contract will be awarded as a personal service, fixed-price type contract to the offeror whose proposal is responsive and responsible. Technical proposals will be evaluated based on numerically weighed technical evaluation factors. Technical and cost/price proposals are of equal value, in the case of a tie of technical proposal ranking, cost/price factors will determine award of contract.
