commercial items prepared in accordance with the format in FAR Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued.
This solicitation F0561199Q0142 is issued as a request for quotation
(RFQ). This solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular 97-07.
The Standard Industrial Classification Number is 7999 and the Business
Size Standard is $5.0M. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON
EACH REQUEST. Women and small disadvantaged business are encouraged to
submit offers. The proposed contract listed here is 100% set-aside for
Small Businesses. Line Item 0001 -- Personal Trainer/Fitness Instructor
as per attached Statement of Objectives (SOO) -- QTY 12 MO -- UNIT
PRICE _______________ TOTAL PRICE _______________. Statement of
Objective (SOO) for Personal Trainer/Fitness Instructor 1. Contractor
and all employees or subcontractors shall at a minimum meet the
following baseline requirements: a. Certified Health/Fitness Instructor
through the American College of Sports Medicine (ACSM), Certified
Personal Trainer or Certified Strength and Conditioning Specialist
(C.S.C.S.) through the National Strength and Conditioning Association
(NSCA), or Certified Personal Trainer/Physical Fitness Specialist
through the Cooper Institute for Aerobics Research, or prior to
contract, be able to demonstrate a combination of equivalent
certifications and a minimum of 5 years experience in the Health and
Fitness Field, have completed at least 2 years (24 semester hours) of
college level courses in Exercise Physiology or Sports Medicine, which
is acceptable to the Director of the Fitness and Sports Center or his
designated representative. b. CPR and First Aid certified. 2.
Contractor shall have the broadest base of expertise with a combination
of certifications and demonstrate experience, acceptable to the
Director of the Fitness and Sports Center or designated representative,
to serve a widest possible range of special populations served by the
Fitness and Sports Center. Quality of service shall be measured based
on contractor's ability to assess, prescribe and instruct physical
conditioning programs for the following members: a. Active duty members
in need of meeting Air Force physical fitness requirements b. Adult
amateur athletics in a broad range of sports c. U.S. Air Force
Prep-School cadets who must meet Air Force Academy entrance
requirements d. Elderly clients from the retired community e. Pregnant
women f. Members in need of weight loss programs g. Patrons in need of
weight loss programs h. Members requesting general health and fitness
improvement 3. Additional education and certifications desirable
include, Associates of Arts Degree or higher in Sports Medicine or
closely related field, certifications by the American Council on
Exercise (ACE), Aerobics and Fitness Association of America (AFAA),
International Sports Medicine Association (ISMA), as well as
certifications or coaching certifications in various sports, and at
least 5 years of experience in the sports and fitness field. 4.
Contractor shall implement individualized and/or group fitness programs
in accordance with American College of Sports Medicine Guidelines for
Exercise Testing and Prescription and/or, in accordance with National
Strength and Conditioning Association Essentials of Strength Training
and Conditioning. Programs/Services shall include but not be limited to
the following: HEALTH RISK SCREENING COMPREHENSIVE FITNESS ASSESSMENTS
EXERCISE PRESCRIPTION WEIGHT ROOM SUPERVISION HEALTH EDUCATION AND
MOTIVATION STRENGTH TRAINING TO INCLUDE FREE WEIGHTS, WEIGHT STACK
MACHINES & CALISTHENICS FLEXIBILITY TRAINING CARDIOVASCULAR TRAINING
NUTRITION COUNSELING WEIGHT LOSS PROGRAMS VARIETY OF GROUP EXERCISE
CLASSES SPINNING and/or AEROBICS CLASSES 5. Contractor and all
employees orsubcontractors must fully meet all the qualifications
required to perform Personal Trainer/Fitness Instructor services, or be
approved by the Director Fitness and Sports or designated
representative. 6. Session times required for trainer/instructor will
be determined upon customer demand with emphasis placed on maximum
coverage of the Fitness and Sports Center hours. 7. Payments to
contractor will be made bi-monthly for actual services rendered and
provided by the contractor and accepted during the preceding week.
Billing will be based on the number of hours of instruction and/or
related work. 8. Contractor will vary his/her hours only at the
discretion of the Director, Fitness and Sports or his designated
representative. 9. Contractor will be responsible for keeping
attendance rosters and recording client's progress charts. 10.
Contractor will have a ninety (90) day probationary period at which
time contractor may be relieved of duty at the discretion of the
Director, Fitness and Sports Center or the designated representative if
not performing up to standards. 11. Instruction will be held at the
Base Fitness and Sports Center, Building 5234, USAF Academy, or other
designated location when approved by the Director Fitness and Sports or
his designated representative. Offerors must comply with all
instructions contained in FAR 52.212-1, Instructions to
Offerors-Commercial, addenda, acceptance time period of 70 days. The
following additional FAR provisions and clauses apply: 52.212-2,
Evaluation-Commercial Item, the Government will award a contract
resulting from this solicitation to the responsible offeror whose offer
conforming to the solicitation will be most advantageous to the
Government, price and other factors considered. The following factors
shall be used to evaluate offers: 1) Technical acceptable; 2) Past
performance; and 3) Price. Technical acceptable is considered the most
important. Technical acceptable will be evaluated based on the
function and operation of the items. To be technically acceptable the
item shall meet the Government's specifications. Offerors shall submit
descriptive literature, product samples, technical features, and
warranty provisions. Past performance is considered more important than
price. Past performance will be evaluated to ensure satisfactory
business practices and timely performance. Price will be considered as
the least important factor. Price will be evaluated to determine its
fairness, completeness, and reasonableness as it relates to the items
offered. 52.212-3, Offeror Representations and
Certifications-Commercial Items, a completed copy of this provision

The following statement apply: IT Statement -- Year 2000 Compliance
Requirement -- All information technology items must be year 2000
compliant, or non-compliant items must be upgraded to be year 2000
compliant by (NOTE: no earlier than 1 Oct 99, but not later than
December 31, 1999). Year 2000 compliant means information technology
that accurately processes dates/time data (including, but not limited
to, calculating, comparing, and sequencing) from, into, and between the
twentieth and twenty-first centuries, and the years 1999, 2000, and
leap year calculation. Furthermore, year 2000 compliant information
technology, shall accurately process date/time data if the other
information technology, shall accurately process date/time data if the
otherinformation technology properly exchanges date/time data with it.
