a combined synopsis/solicitation for Commercial items prepared in
accordance with the format in subpart 12.6 as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. The Request for Quote (RFQ)
is F6550198QP055. The provisions and clauses incorporated are those in
Federal Acquisition Circular 97-05, and it has a standard industrial
code(SIC) of 7629. The services required by this SOW include contractor
shall provide all personnel, equipment, tool, materials, and parts, and
supervision, and other items and services necessary to provide
maintenance, calibration, and repair on: Line item 0001: 4 Ohmeda

Hewlett-Packard Anesthesia Monitor Systems Model ACMS. The contractor
shall provide the repairs necessary to return an inoperable item to a
totally functional state of operation as it was originally intended to
operate. After receipt of a trouble call, the contractor's personnel
shall contact the POC (Point of Contact) and commence repair procedures
within 10 working hours for a routine call, and 4 hours for a priority
call (regardless of the time of day). Service must be restored within
24 continuous hours for routine calls, and 4 hours for priority calls.
In the event that parts must be ordered by the contractor, the
contractor must notify the POC when the parts are ordered and update
status of the repair action at the beginning of every workday.
Documentation of training for the personnel performing maintenance
under this contract will be required by the Contracting Officer. The
Contractor shall provide all tools and test equipment, any machines,
attachments, features, etc. which are necessary for the proper
maintenance of the equipment under this contract. The contractor shall
furnish all replacement parts that are not consumable and shall
provide new, used or remanufactured parts. The contractor shall
guarantee that the replacement parts for all items are available for
the duration of this contract, including options. The Contractor shall
have the service technician continue uninterrupted on all service
calls until the equipment is 100% operational according to the
manufacturer's specifications, unless the parts have to be ordered to
complete the work. Have all requested parts, which meet the
manufacturer's specifications received by the service technician within
one working day, The service technician shall commence work no later
than the second working day to complete the necessary service. Perform
preventative maintenance, and calibrations, and upgrades in accordance
with manufacturers requirements. No additional costs shall be incurred
by nor charged to the government. The following provisions apply
