This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This request for quotation (RFQ), number 98X800539, is for the purchase and delivery of a Sunflower Assets property tracking software package. The Department of Health and Human Services (HHS), Program Support Center, knows of only one source, Sunflower Systems, 1848 Holmes Street, Building E, Livermore, CA 94550. The solicitation document and incorporated provisions and clauses herein are those in effect through Federal Acquisition Circular 97-05. The Standard Industrial Classification code is 5045 with a small business size standard of 100. The Contractor shall provide one (1) each of the following items: 1) Sunflower Assests basic software license; 2) Annual support to include telephone assistance during standard business hours, 8 hours a day, 5 days a week, maintenance releases, and product upgrades; 3) Full training for System Administration and Operators (one week on-site for up to 10 staff); 4) Data conversion services -- convert and migrate the current Property Management Information System (PMIS) to the new database. The extraction of the database file shall be in text format. The Government will furnish the database PMIS data to the Contractor. Delivery of the software is required 3 weeks from the date of issuance of a purchase order. All items shall be delivered FOB destination to the Program Support Center, Personal Property Facility, 16071 Industrial Drive, Gaithersburg, MD 20877. The provision at FAR 52.212-1, Instructions to Offerors -- Commercial Items (AUG 1998), and the clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (APR 1998), applies to this acquisition. Offeror shall submit a fully executed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (JAN 1997) with their offer.
