Administration (FCA) is seeking a contractor to underwrite and
administer a group term life insurance program for approximately 200
employees and retirees. The levels of coverage include Basic (equal to
two and one-half times employees' salary (up to a maximum of
$200,000); Accidental death and dismemberment in an equal amount;
Option 1: Additional coverage on employees in $25,000 multiples up to
a maximum of $300,000; Option 2: Spousal coverage equal to 50 percent
of the employee's basic insurance; Option 3: Dependent coverage equal
to $10,000 for spouse and $5,000 for each dependent child under age 22.
The basic and AD&D coverage will be included under one premium and will
be contributory with employees paying 2/3 of the cost and FCA
contributing the remaining 1/3. All optional coverages will be paid
entirely by the employee. Employees must enroll in the Basic coverage
in order to elect any options. The FCA Life Insurance Program is an
alternative to the Federal Employees Group Life Insurance Program
(FEGLI) which is open to all Federal employees. Employees who enroll in
both the FEGLI and the FCA Program will pay the entire cost of the FCA
basic coverage themselves since the FCA must, by law, pay a portion of
the basic (FEGLI) coverage. At the present time approximately 200
employees and retirees are covered under this program. FCA does not
expect this number to change significantly. The solicitation package
will be available on or about 15 days after the date of this
publication. All responsible sources may submit an offer which will be
considered. Closing date for the receipt of proposals in response to
the solicitation will be 30 days following issuance of the
solicitation, or approximately October 15, 1998. All requests for the
solicitation package shall be in writing, no telephone requests will be
accepted. The solicitation package may be requested by facsimile

All requests must state that the vendor is not currently on the
Consolidated List of Debarred, Suspended and Ineligible Contractors
