commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; bids are
being requested and a written solicitation will not be issued. This
solicitation is being issued as an Invitation for Bid. The solicitation
number is IFB 565-2-99. The VA Medical Center, Fayetteville, NC intends
to award a firm fixed price contract for the removal/disposal of all
regulated medical waste for the VA Medical Center at 2300 Ramsey
Street, Fayetteville, NC for the period of October 1, 1998 through
September 30, 1999 with four one-year options to extend services. The
contract will be for a period of one (1) year with four (4) option
years. The total duration will not exceed five years. All procedures
shall be in accordance with requirements of the EPA, OSHA and any other
applicable Regulation Agency. In addition to regular infectious waste,
smallquantities of pathological waste, chemotherapy waste and outdated
drugs will be generated for removal/disposal. The VAMC, Fayetteville,
N.C. generates approximately 16,000 pounds of medical waste volume per
month. Contractor will utilize off-site incineration method of
disposal. Contractor will furnish compact container system comparable
to or equal to the MWS-30 compact container system by BFI Medical Waste
Systems. [Specifications -- Length approximately 18" (45.72cm), Width
Approximately 18 "(46.99 cm) Height, approximately 24 11/16" (62.7cm)
Empty Weight, approximately 12 lbs. (5.4 kg), Workable Load Rating
approximately 75 lbs. (34.0kg) Capacity approximately 3.7 cu. ft.
(.1051 cu.m.)] Contractor is responsible for bar coding and weighing of
containers prior to transporting to incineration site. An inventory
list including number of containers picked up and number of empty
containers left will be provided on the day of pickup to the Chief,
Environmental Management Service. Contractor will provide an empty
container for each full container removed from the site storage
facility on an exchange basis. Contractor will remove/dispose of
medical waste on a routine basis as needed to meet demand. It is
estimated that removal/disposal will be approximately 2 times per week.
Schedule of remove/disposal will be coordinated with the Chief,
Environmental Management. Reports of each disposal activity will be
prepared to meet Federal Requirements and will be submitted to the
Chief, Environmental Management immediately upon completion of
disposal. Report will include the date of pickup from station, number
of containers disposed, weight of each container and amount charged.
Payment will be made monthly in arrears upon receipt of properly
prepared invoice. A copy of the disposal activity report must be
submitted with monthly invoice in order to certify payment. BID ITEM 1:
Cost of Off-Site Incineration including boxes for the period October 1,
1998 through September 30, 1999. Cost per pound $_____________________x

Provide Cost for removal/disposal of medical waste for each Option Year
