combined synopsis/solicitation for commercial items prepared in
accordance with the format in Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. The solicitation number for
this procurement is DE-RP04-1998AL78875. This solicitation is issued as
an invitation to bid (IFB). This solicitation incorporates provisions
and clauses which are in effect through Federal Acquisition Circular
97-6. This is a small business set aside under Standard Industrial
Classification Code (SIC) 8999, size class $5 Million. Description of
Contract Requirements: Contractor to provide all labor, supplies,
materials, and equipment necessary to support a physical fitness
program at the DOE Carlsbad Area Office, Skeen-Whitlock Building, 4021
National Parks Hwy. Carlsbad, NM. The period of performance for this
contract shall be a one year base period, with four one year options.
The program shall consist of: aerobics (step/low impact) three mornings
a week @ 0545-0645 hours, and three afternoons a week @ 1645-1745
hours; yoga 3 days a week @ 1200 hours; weight training/body sculpting
2 days a week @ 1200 hours; and therapeutic massage (to be available
2 days a week / sessions to be paid by employees). All services shall
be provided Monday through Friday. All instructors and therapists shall
be duly certified or licensed in their respective fields. This
procurement is a commercial item acquisition, using simplified
acquisition procedures, IAW FAR 12 and 13. FAR 51.212-1, Instructions
to Offerors -- Commercial Items, applies to this acquisition. FAR
52.212-2, Evaluation -- Commercial Items, applies to this acquisition.
The evaluation criteria for this procurement will be based solely on
price. All Offerors should include a completed copy of the provision at
52.212-3, Offeror Representations and Certifications -- Commercial
Items, with its offer. FAR 52.212-4,Contract Terms and Conditions --
Commercial Items, applies to this acquisition. FAR 52.212-5, Contract
Terms and Conditions Required To Implement Statues or Executive Orders
-- Commercial Items, applies to this acquisition. The FAR clauses
cited in FAR 52.212-5(b), paragraphs: (6), (7), (8), (9), and (10) are
applicable to this acquisition. The following additional contracts
requirements are applicable to this acquisition: FAR 52.217-9, Options
to Extend the Term of the Contract, with the paragraph (c) being
hereby replaced with the following: "The total duration of this
contract, including the exercise of any options under this clause,
shall not exceed 5 years;" and FAR 52.219-14, Limitations on
Subcontracting. All bids are due by 1630 hours on September 20, 1998.
Bidders may mail, fax, or email their solicitation package and bid for
the required service to the POC listed in this announcement. All bids
submitted by mail shall include the following statement on the
envelope "Mailroom DO NOT OPEN, BID ENCLOSED -- FORWARD TO DOE
