a combined synopsis/request for quotation (RFQ) for commercial items
prepared in accordance with the format in FAR subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; proposals are being
requested and a written solicitation will not be issued. The
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 90-41. This acquisition
is unrestricted. The SIC code is 3534 size 500 employees. Delivery
shall be FOB destination to Andrews AFB, MD. Responses to this RFQ must
be received by 21 August 1998 no later than 12:00 p.m. EST. Delivery
and finance addresses will be provided upon award. Award is to be based
on the following factors: (1) Offerors are required to document past
performance; (2) Ability to complete repair within a specified time
frame and (3) Price. Past performance and delivery time rate
considerably higher than price. PERFORMANCE WORK STATEMENT
Repair/Overhaul of Reach-All 45ft. Aerial Tower Truck, Registration
Number: 86C01624 VIN Number: 1HTLCXN4GHA24355The contractor shall
furnish all parts, labor, equipment, tools, personnel, transportation,
materials, technical supervision, repair reports, and services
necessary to perform all operations in connection with the
repair/overhaul of Reach-All 45ft. Aerial Tower Truck, Registration
Number: 86C01624. The vehicle shall be made available to potential
contractors for the purpose of inspection at 3320 Pennsylvania Avenue,
Andrews AFB, Maryland. All work shall include materials and labor and
will conform to all manufacturers specifications: 1. Perform
operational, dielectric, weight, and hydraulic cylinder leak down tests
before and after repairs of the boom, outrigger, and related
assemblies. 2. Replace upper boom and weldment assemblies. 3. Convert
boom assembly to a single stick control option to include new basket,
basket cover, guards, and liner. New basket will be a single two person
basket mounted centered on the end of the boom assembly and not mounted
to the side. 4. Install new emergency bypass selector valve at lower
controls. 5. Replace boom rotation-bearing assembly. 6. Replace all
boom assembly hydraulic lines. 7. Replace all boom assembly hinge pins
and bushings. 8. Rebuild all hydraulic control valves assembles. 9.
Rebuild all boom hydraulic cylinder assemblies. 10. Rebuild all
outrigger assemblies. 11. Replace basket to boom cover assembly. 12.
Install new basket level assembly. 13. Service complete boom hydraulic
system, to include new fluid and filters. 14. Repair all dents and
scratches on vehicle, prior to paint complete. 15. Complete corrosion
control and paint complete IAW T.O. 36-1-3; Painting, Marking, and
Lighting Requirements for USAF Vehicles and T.O. 36-1-52, Preparation
and Corrosion Treatment of Vehicles. To include inside compartments
also. (Paint to be supplied by 89 TRNS/LGTM) 16. Apply non-skid paint
to the floor of the bed and on top of the body compartments 17. Repair
bench seat in cab assembly. 18. Replace floormat in cab assembly. 19.
Repair all interior and exterior lights. 20. Install beacon light to
left topside of cab assembly. 21. Install a new three light bar
assembly to rear of vehicle. 22. Install (2) new deck lights to top of
body compartment. 23. Install new front tires and tubes. 24. Install
new mud & snow tires and tubes on rear. 25. Replace all boom
identification decals and placards. Period of performance shall begin
immediately upon award of contract. Maximum vehicle downtime shall not
exceed 45 days. Contractor is required to drive the vehicle and work
is to be performed at their location. Coordination for pickup and
delivery of the vehicle will be given with 24 hours advance notice.
Points of contacct will be provided upon award. Manufacture supplied
parts will be used. Government has incorporate FAR 52.228-10, Vehicular
and General Public Liability Insurance. Contractor is to provide as a
minimum proof of Insurance in the amounts of $500,000 for Bodily
Injury, $200,000 for Each Occurrence, and $200,000 for Property Damage.
Contractor shall state in their proposal that they can provide these
amounts of insurance and awardee shall provide proof of insurance prior
to performance. These minimum amounts of insurance are needed in the
event this vehicle is lost, stolen, or destroyed while under their
care. After the repairs are completed, the contractor shall perform
operational, dielectric, and weight tests IAW manufacturers
specifications; T.O 00-20B-5, USAF Motor Vehicle and Vehicular
Equipment Inspection; T.O. 36C-1-4, Dielectric Testing of Insulated
Manlift Devices; T.O. 36-1-58, General Requirements for Repair,
Maintenance, and Testing of Lifting Devices; AFOSH Standard 127 series
and 29 CFR 1910 OSHA. The contractor shall provide a repair report to
the Government's QAE and 89th Transportation Squadron's Maintenance
Control & Analysis section following completion of this contract. All
referenced Air Force technical orders will be made available upon
request. The report shall include as a minimum: 1. Results of all
operational, dielectric and weight tests. 2. Statement of successful
operational, dielectric and weight tests. 3. Statement of all
applicable warranties, to include those transferable. 4. Statement of
all equipment installed, to include but not limited to the following
information: type, manufacturer, part number, model number and rating.
