This is a combined synopsis/solicitation for Commercial items prepared
in accordance with the format in subpart 12.6 as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; quotes are being requested and a
written solicitation will not be issued. The request for Quote (RFQ) is
F6550198QP028. The provisions and clauses incorporated are those in
Federal Acquisition Circular 97-05, and it has a standard industrial
code(SIC) of: 7929. The services required by this SOW include
contractor furnished labor, supervision, transportation, tools,
equipment and minor materials necessary to accomplish the services of
a Music Coordinator and Organist for the Protestant Chapel program.
Contractor will have a minimum of four years experience in church
music. Some background in Protestant theology is required (e.g.
completing college course, adult education courses, liturgical music
workshops, etc). Contractor will coordinate all music activities of the
Protestant program with the Senior Protestant Chaplain. The contractor
will ensure that all musicians are competent and capable of performing
acceptably for those musical services in which they are engaged.
Musical selections provided by the contractor will be appropriate music
for the specific type of worship service and the Church season. It will
meet the standards of the Senior Protestant Chaplain and the chaplain
conducting the service. It is the responsibility of the contractor to
insure that any subcontractors providing services under this contract
conduct themselves in a professional manner. Each person will arrive no
later than 15 minutes prior to the designated starting time of the
service and will not leave until the service has ended. Religious
services will be approximately one hour in length. All persons engaged
by the contractor for the purpose of providing music service under
this contract will be neatly attired and well groomed. All civilian
attire will be in good taste and of a semi-formal nature. Shoes must be
worn at all times. Sport shirts will be worn inside trousers except for
those shirts designed to be worn outside trousers. Any musician
reporting for duty giving the appearance of having recently consumed
alcohol will not be allowed to perform. Contractor is responsible for
ensuring the chapel facility is left secured and clean after the
rehearsals. The location where services will be performed may change to
accommodate special or unforeseen circumstances. Any substitute or
subcontractor organist/choir director/pianist will be of substantially
equal competence as the contractor. Notice of substitution will be
given to the Senior Protestant Chaplain or his designee 24 hours prior
to the scheduled activity, whenever possible. Approval by the
contracting officer of the substitution is not required. Contractor may
be needed to perform at other services during the period of the
contract, including weddings, funerals, memorial services, and
religious holidays at the same rate per service. Although in normal
circumstances wedding fees are paid by the couple or family. Contractor
will carefully maintain a system for timely compensation of individuals
providing musical services or rehearsals for special services. This
information will be used for accounting and tax purposes. Invoices,
accompanied by the actual provider's certification of nonpersonal
service, will be submitted for processing to the NCOIC, Chapel Resource
Management no later than the first workday of each month. Payment will
be processed upon receipt of certified invoices. Interview and
approval by the Senior Protestant Chaplain is required. The Senior
Protestant Chaplain is appointed as Quality Control Officer to the
Contract and will administer this purchase order for 3 WG/HC, Elmendorf
AFB, AK 99506-2630. Potential contractor and subcontractors will also
be required to submit a signed release/consent statement authorizing a
background check IAW para D., section 1 of Enclosure 6 to Department
of Defense Instruction 1402.5 Failure to positively complete this
background check will result in termination of contract or subcontract.
The Choir Director will provide services on the following days and
times: 52 ea. Performances and 52 ea. Rehearsals for Saturdays at 1700,
52 ea. Performances and 52 ea. Rehearsals Sundays at 0945 and 52 ea.
Performances and 52 ea. Rehearsals on Sundays at 1700. The Organist
shall provide services on the following days and times: 52 each
Performances and 52 ea. Rehearsals on Saturdays at 1700, 52 ea.
Performances and 52 ea. Rehearsals on Sundays at 0924, and 52 ea.
Performances and 52 ea. Rehearsals at on Sundays at 1700. Special
Services will be: 38 each for (Funerals, Memorials, Holy Days, etc.).
The following provisions apply 52.212-1, Instructions to Offerors,
Commercial Applies to this acquisition and any addenda 52.212-3,
Advising Offerors to include a completed copy of representation and
certifications-commercial items with offer, 52-212-4, Contract terms
and Conditions-Commercial items, applies to and a statement regarding
any addenda to clause, and 52.212-5, Contract terms and conditions
required to implement statues or executive orders- Commercial items,
