Contracting Squadron Elmendorf AFB AK, gencoCW@Elmendorf.af.mil. This
is a combined synopsis/solicitation for Commercial items prepared in
accordance with the format in subpart 12.6 as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; quotes are being requested and a
written solicitation will not be issued. The request for Quote (RFQ) is
F6550198QP048. The provisions and clauses incorporated are those in
Federal Acquisition Circular 97-05, and it has a standard industrial
code(SIC) of:7991. The services required by this SOW include contractor
furnished labor, supervision, transportation, tools, equipment and
minor materials necessary to accomplish Instructor Weight Management
Program. SCOPE OF WORK: Contractor shall conduct and administer all
aspects of the exercise portion of the Weight Management and Fitness
Enhancement Programs held in the Base Fitness Center at Elmendorf Air
Force Base, Building 4-940. PERSONNEL: The contractor or designated
representative shall meet with government personnel designated by the
Contracting Office to discuss immediate problems. The Contractor or
representative shall respond within four hours after notification. The
contractor shall provide the telephone number of the person(s) to call
should the need arise. EMPLOYEE IDENTIFICATION: Contractor personnel
must be recognizable as such while in the Fitness Center. This should
be accomplished by wearing distinctive athletic attire. The contractor
is responsible for acquiring and maintaining an appropriate number of
uniforms to meet their need at their expense. SPECIFIC DUTIES:
Position is to manage, supervise, and conduct the Elmendorf Air Force
Base Weight Management Program and Fitness Enhancement Program.
Contractor will be responsible for the following: Individual must
provide to the Contracting Office personnel documents certifying
accreditation in physical conditioning, weight management, or
certification from other recognizable institutes. As a minimum the
individual must be certified by a nationally accredited organization as
an aerobic instructor(i.e. ACE, Dante). Classes will consist of an
aerobic type workout. Classes should be well rounded and include, but
not limited to: step aerobics, slide aerobics, high impact, low impact,
modified, and pre-natal instruction. NOTE: At least one class taught
during the day must have a person trained to give advise and
instruction in pre-natal programs. The contractor will submit a monthly
schedule to the Athletic Director for approval. The schedule will
include the instructors name, type of class, and time. Must keep
current and provide proof of certification in Cardiopulmonary
Resuscitation as outlined by the American Medical Association or the
American Red Cross. Contractor will perform classes using the following
schedule: October classes at 0600, 1130, and 1730 hrs Monday -- Friday.
Nov -- Feb classes at 0600, 1130, 1715, and 1830 hrs Monday -- Friday.
Mar -- Sep classes at 0600, 1130, and 1730 hrs Monday -- Friday. These
times are subject to change based on the needs of the active units.
Contractor will be notified 30 days in advance of any changes in class
times. The program is outlined in AFI 34-107 Section B, paragraph 10.
Contractor shall provide all tapes and music requirements for the
classes. Contractor will ensure that all equipment (steps, mats, bands,
music equipment etc.) is used according to the Fitness Center's
policies and are properly put away by the students or instructor after
class. The contractor will handout at least 10 customer comment cards
per day as provides by the fitness center. The contractor shall be
financially responsible for any damage caused to fitness center
equipment while it is under their control (i.e. stereo system,
microphone/headset, ect.) to include repair/replace, maintenance,
shipping and handling of damaged equipment. The contractor will record
an accurate headcount for each class performed using forms provided by
the fitness center. The contractor will perform a check of students
heart rate at least three times per class. The contractor will limit
the number of classes taught by one instructor to no more than two
classes per day. The following holidays will be recognized and no
instruction will be necessary: New Year's Day, Thanksgiving Day,
Christmas Day and New Year's Day. Classes will be taught on Good
Friday. The following provisions apply 52.212-1, Instructions to
