[This Commercial Item, non-personal services contract incorporates
provisions & clauses from FAC 97-04 (#62) and VAAR FR-4/9/98 (#17).

Services shall be provided in accordance with the requirements of the
contract to include, but not be limited to the following: 1. General
Diagnostic Services, Cross Sectional Imaging Services, Ultrasound, and
Nuclear Medicine Services. a. Administer, direct, perform and
interpret : cross sectional imaging services in the area of Computer
Tomography (CT), Ultrasound, General Nuclear Medicine, General
Diagnostic Radiology -- to include Fluoroscopy of the gastrointestinal
tract, IVPs, barium enemas, mammography, routine diagnostic studies of
the abdomen, chest and musculoskeletal system.b. Communicate pertinent
patient data to charge physician, as appropriate. Shall be responsible
also for entering information into computer, retrieving information,
some clerical duties in connection with position, direct patient
contact as well as telephone contact. c. Recognize and initiate
appropriate measures in emergencies including operation of
resuscitation equipment.. Possess knowledge of actions and usual dosage
of most commonly used emergency drugs and their usage. d. Participate
in facility provided training, to include fire drills, fire & safety
training, and any other facility training required by Chief of
Radiology Service. Shall also attend Service staff meetings as a part
of required duties. e. Prepare reports, assure content and correctness
of prepared reports and sign documents to validate their contents. f.
Provide professional direction and teaching of clinical staff . g.
Teach, lecture, and direct technologists in all contracted areas of
radiology. Contractor personnel must be present at the VA Medical
Center and actually be performing services required by the contract
during the scheduled work period to be paid for that period. Payment
will be made only for actual hours in which services are rendered.
Monthly invoices must be accompanied with copies of VA verified time
sheets. Holidays which are not worked are not paid. Contract employees
will be required to sign in and out utilizing the approved time
sheets. Cost quoted in the proposal is to be all-inclusive of all
incidental cost including housing, transportation, taxes, overhead, and
premium payments for applicable malpractice insurance coverage for
Contractor's personnel. Amount to be paid by VA will be a flat hourly
fee bid on in Section B. This contract represents a firm-fixed price
contract. Services performed outside the scope of this contract are not
authorized and will not be paid. Contractors will be required to
provide information needed for VA credentialing and privileging.
Candidates must be clinically privileged at the Las Vegas VA Medical
Center prior to providing services. This process will be completed in
accordance with the National Practitioner Data Bank (NPDB) as
established by PL 99-660. Completion of credentialing and privileging
packets will be required for contract employees once the determination
has been made that the proposed physician(s) have met the
qualification criteria and the apparent awardee has been determined.
The Contractor shall assume full responsibility for the protection of
its personnel furnishing services under this contract in accordance
with the personnel policy of the Contractor, such as providing worker's
compensation, professional liability insurance, health examinations,
income tax withholding, social security payments, sick leave and
vacation time. Such personnel will not be considered VA employees for
any reason. The Contractor shall ensure that all contract personnel
have met qualifying criteria, including current and valid licenses and
certifications. The qualification of such personnel shall also be
subject to review by the VA Chief of Staff and approval by the VA
Facility Director. Contract Radiologist shall be subject to the same
quality assurance standards as established by the Joint Commission for
Accreditation of Healthcare Organization (JCAHO). Persons provided by
the Contractor shall be technically proficient in the skills necessary
to include the ability to speak, understand, read and write English
fluently. Contractors are responsible to review clauses & references in
the solicitation. Upon request, the Contracting Officer will make the
full text clause available. The offeror is cautioned that the listed
provisions may include blocks that must be completed by the offeror and
submitted with its quotation or offer. In lieu of submitting the full
text of those provisions, the offeror may identify the provision by
paragraph identifier and provide the appropriate information with its

Commercial Items. (OCT 1997) (a) The Government will award a contract
resulting from this solicitation to the responsible Offeror whose offer
conforming to the solicitation will be most advantageous to the
Government, price and other factors considered. THE FOLLOWING FACTORS
SHALL BE USED TO EVALUATE OFFERS:AWARD MAY NOT BE TO THE LOWEST OFFERED
PRICE. THE VA RESERVES THE RIGHT TO MAKE AWARD WITHOUT ENTERING INTO
DISCUSSIONS WITH ANY OFFEROR INFORMATION REQUESTED BELOW MUST BE
FURNISHED FOR EVALUATION BY THE GOVERNMENT. FACTOR 1 -- PERSONNEL
QUALIFICATIONS/EXPERIENCE: Radiologists (Board Certified by the
American Board of Radiology) assigned by the Contractor to perform the
services covered by this contract shall be licensed in a State,
Territory or Commonwealth of the United States or the District of
Columbia. Personnel must possess experience in cross sectional imaging
modalities as demonstrated by special training such as having
completed a one year fellowship training in CT and ultrasound or as
clearly demonstrated by practice experience to be verified by
references. Shall be trained and experienced in General Diagnostic
Services. Personnel must have one year experience in general diagnostic
to include doing fluoroscopy of the gastrointestinal tract, IVPs,
barium enemas, mammography, routine diagnostic studies of the abdomen,
chest and musculosketetal system. Must also have sufficient experience
in Nuclear Medicine, as determined by the facility's Professional
Standards Board, to perform and interpret routine nuclear medicine
studies. The qualifications of such personnel shall also be subject to
review by the VA Chief of Staff and approval by the VA Facility
Director.- Supply complete resumes (curriculum vitae) for each
physician being proposed to furnish services under the terms of this
solicitation. Include back-up personnelinformation.- Provide three (3)
recent references for each physician being proposed from which this
facility will be able to certify current clinical capability.- For each
physician proposed, submit a copy of current privileges candidate has
at any hospital in which they have staff membership.- Those physicians
who have already provided these exact services for this Medical Center
need not resubmit credentials if they are still current.- Certify
ability to begin performing duties on this contract within five (5)
workdays of award.- An SF-129, Solicitation Mailing List Application,
is enclosed with this Request for Proposal and must be completed and
returned with your proposal.- An Electronic Funds Transfer Payment form
is also enclosed with this Request for Proposal and must be completed
and returned with your signed proposal. FACTOR 2: COST/PRICING The cost
stated in Section B is to be inclusive of all required services for the
term of the contract. Award will be made to the Offeror who attains the
highest combined evaluation of all factors, including the offered
price. Therefore, the Offeror with the lowest cost or price alone may
not necessarily be awarded the contract. The VA reserves the right to
award without discussions, based on evaluation information only. It is
therefor e recommended that all Offerors submit their best proposals
for the initial evaluation phase. FACTOR 3: PAST HISTORY/EXPERIENCE
(MANAGEMENT QUALIFICATIONS) 1. Provide a minimum of three ( 3) recent
references that may be contacted, to whom you have provided
Radiologists. Include the name of a Contact Person, telephone number,
quantity and type of Radiologists. 2. Number of years in business, to
include breakdown of number of years in area. Include any information
on offices/branches, their locations, number of years in location and
reference information for those branches. 3. Agency enforcement of
standards of review and evaluation to ensure ongoing quality service
from health care professionals. 4. Supply a copy of your hiring
policy/criteria for Radiologists. 5. Provide credentialing and
privileging policy. 6. Provide a list of previous contracts (within the
past five (5) years) showing ability to provide Radiologist services,
to include, but not be limited to: a. Approximate dates by year. b.
Names of organizations provided these services. c. Number of
Radiologists provided (include area of specialty) 7. Provide
contingency plans to be used in the event of: a. Unexpected absences b.
Required replacement due to rejection of Contract Personnel. 8. Outline
current compensation structure utilized to obtain and keep suitable
qualified personnel. 9. An apparently successful Offeror, upon request
by the Contracting Officer, shall furnish prior to contract award
evidence of the insurability of the Offeror and/or of all Radiologists
who will perform under this contract. NOTE: Personnel
Qualifications/Experience (Factor 1) & Past History/ Experience (Factor
3), when combined, are slightly more important than price (Factor 2).
