B -- HEALTH CARE ASSESSMENT PROJECT CENSUS SOL RFP 518-03-98 DUE 092398 POC Elaine Gibson, Contracting Officer (90C) Contractor is to develop an outcomes management system that monitors patient-centered outcomes by assessing functional status. Conduct a national level census of all veterans who have been enrolled in the VA health care system. Census is to be conducted over a short time frame (8 weeks) after October 1st 1998 (FY1999) and will provide baseline data of veterans users for clincal monitoring and actuarial purposes, all in accordance with the work statement. Estimated veteran population is 3.5 million. Offerors will provide seperate technical and pricing proposals. The technical evaluation factors are listed in descending order of importance: 1. Technical capabiity of the item offered to meed the Government requirement. 2. Personnel qualifications. 3. Past performance. 4. Prior experience with contracts of this magnitude. 5. Management capabilities. 6. Price/Cost. Technical proposals shall not include prices or pricing information. The Government may make a final determination regarding a proposal's acceptability solely on the basis of the proposal as submitted and the Government may proceed without requesting further information from any offeror, however, the Government may request additional information from offerors of proposals that it considers reasonable susceptible of being made acceptable and may discuss proposals with their offerors. Only one techncial proposal may be submitted by each offeror. Performance requirements are 8 weeks after award of contract. The information is not binding on the Government and the actual delivery or perfomance requirements will be contained in the solicitation. The issue date for this project August 25, 1998.
