synopsis/solicitation for a commercial item prepared in accordance with
the format in Subpart 12.6, as supplemented with additional information
included in this notice. IMPORTANT NOTICE: DFARS 252.204-7004 "Required
Central Contractor Registration" requires registration in the CCR

IT'S MANDATORY TO OBTAIN A DUNS NUMBER PRIOR TO REGISTERING. CALL DUN
& BRADSTREET (1-800-333-0505) TO OBTAIN. AWARD CANNOT BE MADE UNLESS
REGISTERED. This announcement constitutes the only solicitation;
proposals are being requested and a written solicitation will not be
issued. Solicitation No. F08650-98-T-9981 is issued as a Request for
Quotation. The solicitation document and incorporated provisions and
clauses are those in effect thru Federal Acquisition Circular 97-05 and
thru Defense Acquisition Circular 91-13. This solicitation is 100%
set-aside for small business. It is advertised with a Standard
Industrial Classification Code (SIC) of 8049, with a size standard of
$5 million. Contractor shall furnish all labor, material, music,
transportation, and all other items necessary to provide training on
proper nutrition for 10 to 30 personnel. Training will be held in Bldg.
546, Health and Wellness Center, on Patrick AFB FL from 01 Oct 98 -- 30
Sep 99. Dates and times to be mutually agreed upon, but not later than
fourteen (14) days prior to start of class. Variation locations will
be coordinated fifteen (15) days in advance and remain within Brevard
County, Florida. Following is a list of program requirements and hours
of services required: Diabetes Education: 6 classes @ 20 hours per
class/120 total hours per year; Cooking Demonstration: 20 classes @ 2
hours per class/40 total hours per year; Wellness Clinic: 6 classes @
2 hours per class/12 total hours per year; Weight Management: 4 classes
@ 44 hours per class/176 total hours per year; Nutritional Education
(One hour sessions) Choose to Lose (Lipid Management Program): 16
classes @ 5 hours per class/80 total hours per year. This contract will
be for one basic year and two option years. (Line Item 0001) Est.
Quantity: 858 HRS; basic performance period 1 Oct 98 through 30 Sep 99;
(Line Item 0002) Est. Quantity: 858 HRS; Option Year I, performance
period 1 Oct 99 through 30 Sep 00; (Line Item 0003) Quantity: 858 HRS;
Option Year II, performance period 1 Oct 00 through 30 Sep 01. The
following provisions and clauses apply to this acquisition: FAR
52.212-1 Instructions to Offerors -- Commercial Items is hereby
included by reference. The following are addenda to FAR clause
52.212-1; At paragraph (c), Period of Acceptance for Offers, the amount
of time an offeror must hold its prices firm is hereby changed. An
offeror must hold the prices in its proposal, firm for 60 calendar days
from the date specified for receipt of proposals. Paragraph (e)
multiple offers has been determined by the Contracting Officer to be
non-acceptable to this solicitation and is hereby removed in its
entirety for the purpose of this requirement. Offerors are advised that
proposals made be evaluated without discussions. Award might be made to
the responsible offeror whose proposal is responsive to the terms of
the RFQ and is most advantageous to the government. In accordance with
FAR subpart 9.104-1, each offeror is required to provide information
on a reasonable number of recent and relevant contracts for the same or
similar items, as well as the offeror's primary financial institution,
to include contract numbers, points-of-contacts, telephone numbers,
and any other relevant information, which will be used by the
Contracting Officer to make a responsibility determination about a
prospective offeror. Far provision 52.212-2 Evaluation -- Commercial
Items applies to this solicitation and the following evaluation
criteria (arranged in their relative order of importance) are hereby
incorporated: Award will be made to the offeror who meets the
solicitation's minimum criteria for technical acceptability with
regards topast performance at the lowest price. To be determined
technically acceptable, the offeror must show proof they are a
registered dietitian and certified diabetes educator. FAR provision
52.212-3 Offeror Representations and Certifications -- Commercial Items
shall be completed and submitted with the proposal. FAR clause 52.212-4
Contract Terms and Conditions -- Commercial Items hereby applies to
this solicitation. The following are addenda to FAR clause 52.212-4.

government intends to make a single award to the responsible offeror
whose offer is the most advantageous to the government considering
