Hayden VA Medical Center requires primary care services for eligible veterans. The provider will provide for the delivery and management of primary medical care. Referals for sub-specialty care and hospitalization will be made at the Phoenix VA Medical Center. To be considered for award, the offeror must (1) have existed as a healthcare provider business entity providing similar services for a minimum of two years as of the date of submission of its proposal, (2) provide certification that all providers are licensed by the appropriate governing body, (3) have a minimum of three facilities located within the deliniated area. Compensation will be made through a monthly capitation payment. It is expected that services will begin within 30 days of contract award. The term of the contract will be for one year.
