The Navajo Area Indian Health Service is soliciting proposals from interested board certified urologists to provide urology services to Indian patients at the Gallup Indian Medical Center, Gallup, NM. The Contractor shall provide all necessary personnel, transportation, malpractice insurance and shall be licensed in a state. The estimated workload it two (2) to three (3) major cases per visit (prostatic and renal surgery), and an estimated four (4) minor cases per visit (cystoscopies, TURP, etc.) The estimated annual visits are 50 visits. Contract to be effective for a twelve (12) month period with two (2) one-year option periods. Interested offerors should submit their request for a copy of RFP M98-0030/PD.
