Provide staffing to allow for case acceptance with immediate emergency resonse 24 hours day, 7 days a week access. Services to be provided: A. Professional Assessment & Referral to existing cmmunity resources. B. Alchol & Chemical Dependency Recovery Services. C. Drug Free Work place program and reporting. D. Specialized Children's Services. E. Phychiatric Consulttion & Assessment. F. Stress Reduction Services. G. Counseling: a. Individual Pshchological. d. Career. b. Marriage & Family. e. Conflict. c. Financial. f. Grief & Counseling. 2. Provide up to seven counseling session if management referred. 3. Provide up to four confidential counseling sessions per identified problem for each eligible person per year. 4. Provide address of offices maintained or used in the vicinity of NAS Lemoore, CA and Fallown NV. 5. Provide number of staffing persons, full-time and their titles, degrees and certifications. 6. Maintain records and provide two semi annual reports. 7. Cost will be a flat fee per employee per month. If and when the services above are requested, a Quotation (RFQ) will be submitted with further detail information and (SOW) Statement of Work would be provided.
