This action originally publicized in the CBD dated 4/29/98, is amended
as follows: The delivery date has been extended to: June 5, 1998,
C.O.B. 5:00pm E.S.T. The U.S. Department of State requires the
following: 1. Contractor must supply at least one example of a past
performance evaluation demonstrating that they have presented a similar
Health Training Seminar of equal size and scope within the last 5
years. 2. Failure to demonstrate that a similar course was offered
within the last 5 years, and/or failure to submit proper resumes on all
participating Trainers conducting the class, would grant the
proposal/submission ineligible. 3. All workshop trainers must be
licensed or have a Masters Degree in the field of discussion or be able
to demonstrate specialized experience in their discipline. 4. A
licensed nutritionist must conduct the Nutrition portion of the
training session. 5. The U.S. Department of State reserves the right to
verify this information. 6. No Travel Expenses/Per Diem will be paid.
7. At this time, the US Department of State does not know whether a gym
will be made available to the seminar participants. 8. Contractor will
conduct/perform the medical screening test/cholesterol test using a
licensed medical technician at a minimum. 9. Contractor will provide
workshop course material. 10. Contractor will provide certificates for
completion of the course to students. 11. The number of hours for the
class are as follows: 4-8 hour sessions (32 hours) with independent
exercise sessions and/or homework which would consume an undetermined
amount of time, (to be determined later). Perhaps in the two week
period which would extend the session into a 6 week course. 12. Provide
an original and one copy of your bid/proposal. Please refer to
