719-333-2570 17. This is a combined synopsis/solicitation for
commercial items prepared in accordance with the format in FAR Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued.
This solicitation is issued as a Request For Quote (RFQ), solicitation
number F0561198T1680. This solicitation document and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-3. The Standard Industrial Classification Number is 8011
and the business size standard is $5 million. The proposed purchase
order is unrestricted. Women owned small businesses are encouraged to
submit proposals. The Air Force Academy intends to contract for the
one-time services of a Locum Tenens board certified diagnostic
radiologist to perform any and all ultrasound and mammography
examinations in support of the Breast Cancer Prevention, Education, and
Diagnosis Initiative. Required examinations include but or not limited
to: ultrasound of the abdomen, breast, head and neck, pelivs including
obstetrical and transvaginal, peripheral (popliteal fossa) and
shoulder, thorax, scrotum, prostate, doppler color flow studies,
ultrasound guided cyst aspirations stereotactic biopsies, core biopsies
and all radiographic examinations of the breast (mammography).
Performance period will be 5-days per week, Monday through Friday,
eight hours per day, from 0730 A.M. until 1630 P.M. with one hour for
lunch, during 22 JUN 98 through 31 JUL 98, for a total of 240 hours.
This person must be English speaking, board certified diagnostic
radiologist, technically qualified and authorized to practice medicine
in the state of Colorado, and current for mammography interpretation
in accordance with the American College of Radiology requirements. The
contractor will provide round-trip transportation to work site, car
rental, lodging, credentialing, licensure costs, and malpractice
insurance. LINE ITEM 0001: Basic Period: Quantity: 240 hours (hr). Unit
Price: $__ ___. Extended Amount: $__ ___. Services to be performed at
the USAF Academy Hospital/SGSL, BLDG 4102, USAF Academy, CO 80840
(F.O.B. destination). ALERT: Vendors are STRONGLY encouraged to use the
Standard Form 1449 as the first page of their offer/quote/bid and
completely provide the requested data for all blocks. If you choose NOT
to use the Standard Form 1449, you are cautioned to complete and
provide with your written offer/quote/bid all the data requested by FAR
52.212-1. Failure to do so may render your offer/quote/bid as
non-responsive and may eliminate you from consideration for award. The
clause at 52.212-1 applies to this solicitation; however, no addenda
applies to 52.212-1. The provision at 52.212-2 Evaluation -- Commercial
Items applies to this acquisition with all offers being evaluated
against the following factors in the descending order of importance: 1)
Technical capability/ Past performance, and 2) Price. Technical
capability of the proposed items offered shall be evaluated to
determine if the credentials offered meet the specifications set forth
in this solicitation. Past performance will be evaluated to determine
if the offeror has supplied required services, with similar terms
within the past six months. Offerors must provide with the proposal at
least three (3) references to illustrate this past performance for the
purpose of evaluation. Offerors failing to meet the technical
capability/ past performance evaluation will be eliminated from
consideration. Price will be evaluated for fairness, reasonableness,
and completeness as it relates to the services offered. Award will be
made based on a technically acceptable/low price basis. Offers shall
include a completed copy of the provision at 52.212-3, Offeror
Representations and Certifications -- Commercial Items, with its offer.
The clause 52.212-4, Contract Terms and Conditions -- Commercial Items,
the clause at 52.212-5, Contract Terms and Conditions Required To
Implement Statutes or Executive Orders -- Commercial Items applies to
this acquisition. Additional FAR clauses cited in the clause applicable
