Census 2000 Short Form Questionnaire Packages: Approximately 120,000,000 total packages. Each package includes: One questionnaire imaged with a mailing address and a return address, one letter, and one return envelope all inserted into an outgoing envelope. Questionnaire Specifications: A folded form, 22 x 10-3/8" flat and 5-1/2 x 10-3/8" folded using two parallel folds, printing in 2-colors on the face and back. Possible questionnaire changes are: Flat Size 25-1/2 x 11"; Folded Size 9 x 5-1/2" (using two parallel folds and one right-angle fold). -- or - Flat Size 25-1/2 x 10-3/8"; Folded Size 5-1/5 x 10-3/8" (using four parallel folds). Approximate Schedule: Award -- -- -- -- -- -- - December 1998 Print files -- -- -- -- - April 1999 Address Files -- -- September 1999 Mailing -- -- -- -- -- -- March 2000 Please respond to the following questions: 1) How late in your production process would you have to know which size questionnaire would be required? 2) If the Government required production of BOTH 120,000,000 of the 22 x 10-3/8" questionnaire AND 120,000,000 copies of just one of the larger size uestionnaires: a) Would you have the capacity to produce both questionnaires (i.e., material, storage, etc.)? b) How late in your production process would you have to know if the Government requires both? c) What are the risks associated with producing both questionnaires, taking into consideration the schedule indicated above? d) What effect would producing both questionnaires have on the above schedule? 3) Would you address in line during the printing process -- or -- would you address during the inserting operation?
