Provide dental hygiene services at the Coast Guard clinic located at Astoria, OR. to beneficiaries referred for treatment by Coast Guard physicians. This will be a non-personal services contract and medical liability insurance in the amount of $1.0 million is required. This requirement will be accomplished utilizing Best Value Commercial Item simplified acquisition procedures. The contractor will work weekdays during hours established by the Clinic Supervisor, 1 day per week, 8 hours a day, 416 hours per year. Period of performance will be 01 October 1998 through 30 September 1999 with three option years. The Request for Quotation will be issued on or about 15 June 1998 and close on or about 01 July 1998. Performance is expected to begin on or about 01 October 1998. This procurement is a Small Business and Labor Surplus Area Set-Aside. The SIC code is 8049 and the small business size standard is $5.0 million average annual receipts for the preceding three fiscal years.
