Q -- PRIMARY/PREVENTIVE MEDICAL CARE SOL RFP 529-15-98 DUE 051598 POC Sharon Leszczynski 724-477-5052 Provide primary/preventive medical and continuity of care for approximately 850 Veterans Affairs (VA) enrolled patients on an outpatient basis in Armstrong County, PA. Provider must be located within Armstrong County, PA. Contractor to provide health care providers, medical facilities, medical equipment and supplies, and all administrative functions to fulfill the level of care and range of services necessary to meet the primary needs of the enrolled patients. Offerors are being solicited on a capitated basis and will represent an "all inclusive" reimbursement for routine delivery of primary health care service. Contract term will be for a one year base period with two one year options to extend services. Offerors who have the qualifications and capability to provide quality medical care services are invited to submit written requests to the Contracting Officer, reference the RFP number and include the firm's name, address, contact person and telephone number no later than Close of Business 5/15/98.
