Q -- DENTIST SERVICES TO BE PERFORMED ON OUR SITE, FOR 310 TRAINEES, AGES 16 THROUGH 24 SOL HS98-1 DUE 053098 POC Brad Bruton, Purchasing Agent, (909) 887-6305 ext. 205. Requires the services of a licensed dentist in the State of California to work on our site for a maximum of 16 hours per week using our facilities, equipment and supplies. Must be a graduate from an accredited dental school. Provide dental services for trainees ages 16 through 24. Includes exams, routine restorative care, effective referrals for special care and participating in dental health education programs. MTC provides a dental assistant. Must have valid dentist license issued by the State of California, and a valid controlled substance registration certificate issued by the U.S. Drug Enforcement Agency. Must have liability and malpractice insurance. This site is a U.S. Department of Labor Job Corps Center operated under contract to MTC, with a trainee population of 310. Call this center for a copy of the dentist subcontract document. All services to be performed in accordance with Job Corp's Policy & requirements Handbook (PRH), Chapter 6, and Technical Assistance Guide (TAG) #6. The dentistwill be a subcontractor to MTC. Submit bid as an hourly rate. MTC does not provide any form of benefit package. One year contract with option renewal second and third year upon the approval of MTC. All responsible dentists may submit a bid which will be considered by MTC. (0124)
