commercial items prepared in accordance with the Federal Acquisition
Regulation (FAR) sub part 12.6., as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a separate
written solicitation will not be issued. Failure to provide all
required information contained herein may result in the rejection of an
offer. Solicitation number VA-797-98-F2Q-0014 is issued as a Request
for Quotation. The solicitation document and incorporated provisions
and clauses are those in effect through FAC 97-04. The SIC code is 3841
and the small business size standard is 500. The contractor shall
provide the following: 6 each #3626 4.0mm x 30 Video Arthroscope Light
Post Opposite Direction of View; 2 each #3628 4.0mm x 70 Video
Arthroscope Light Post Opposite Direction of View; 12 each #3869 5.8mm
High Flow Cannula Double-Valved, Rotatable; 6 each #3807 5.8mm Conical
Obturator; 6 each #7205377 5.8mm Trocar, Sharp Tip; 2 copies each of
operation and service manuals. These are Smith & Nephew Endoscopy
products. Offerors shall include a 1-year warranty. Place of delivery
is to Wright Patterson Air Force Base, OH. Delivery, which shall be FOB
Destination, shall occur within 14 calendar days after award of the
purchase order. Offerors responding to this announcement shall review
and provide the information contained in FAR clause 52.212-1,
Instructions to Offeror -- Commercial Items and shall include a
completed copy of provision 52.212-3, Offeror Representations and
Certification -- Commercial Items. An authorized representative of the
offeror must sign these representations and certifications. The
Offeror shall also provide its Commercial and Government Entity (CAGE)
code, Contractor Establishment Code (DUNS number) and Tax
Identification number. Clause 52.212-2, Evaluation -- Commerical Items,
applies to this acquisition. The Government will award a contract
resulting from this synopsis/solicitation to the responsible offeror
whose offer conforming to this synopsis/solicitation will be most
advantageous to the Government, price and other factors considered. (1)
Technical capability, (2) past performance, and (3) price. Technical
and past performance when combined are significantly more important
than price. Offerors shall provide technical information, descriptive
literature detailing the products the offeor proposes to furnish as to
materials, components, performance characteristics, etc., and a
minimum of three past performance references, preferably government
references. The resulting firm fixed price award will incorporate the
requirements of the clause at FAR 52.212-4, Contract Terms and
Conditions -- Commercial Items. Clause 52.212-5, Contract Terms and
Conditions Required to Implement Statutes or Executive Orders --
Commercial Items, applies to this acquisition. Offers are due by 2:00pm
EST on May 15, 1998 at AFMLO/VASS to the address shown in this
