information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. Solicitation Number S-OPRAQ-98-M-1450.

F.O.B. Destination to U.S. Department of State, Foreign Affairs
Training Center, 4020 Arlington, Blvd., Rosslyn, VA 22204-1500. Only
one award will result from this solicitation. The award will be made to
the lowest priced, technically acceptable, responsible offer. This
procurement is set-aside for small business. Signed and dated offers

All responsible sources may submit a proposal which shall be
considered. Statement of Work: The contractor shall develop a workshop
for 30 participants which is targeted at providing a personal wellness
approach which will include stress management, health and nutrition,
and fitness. The Office of Medical Services and the Bureau of
Personnel, Office of Employee Relations, through a collaborative
effort, want to present a workshop series on these topics in an effort
to respond to the increasing numbers of stress-related cases in the
Department. Workshop would be presented over 6 consecutive weeks (32
hours), on-site at the National Foreign Affairs Training Center, in
Arlington, VA. The breakdown of the 32 hours isas follows: 2 hours for
the Introduction, 17 hours on Stress Management, 8 hours on Nutrition,
2 hours on Fitness, 2 hours on Changing Behaviors, 1 hour for the
Wrap-up, 32 hours: 4 one-day sessions, 8 hours (9:00 a.m. to 4:30 p.m.
including 1 hour lunch). Times include scheduled breaks. Participants
would be required to take the entire workshop series, as well as
fulfill out-of-class assignments. Program should include assessment,
steps for personal change and follow-up or evaluation. Program must
include instruments or tests for evaluating participants and include
individual stress levels. Workshop segment and moderator should be
presented by an authority on subject(s). Work Requirements Workshop
should be structured for employees at grades 7-12 levels (or FS
equivalent). The combined approach should include modules on stress,
nutrition and fitness. The stress portion should include techniques for
both identifying and reducing stress. Fitness portion would include an
initial (baseline), ongoing and post-fitness assessment of
participants. The baseline medical profile, e.g. blood pressure,
percentage of body fat and cholesterol is to assist students in
establishing their current status and to help them identify areas for
improvement so as to make lifestyle changes. Workshop should include
lectures, exercises, demonstrations, supervised exercise and individual
consultations. Delivery Dates : Proposals should be submitted by COB
May 22, 1998. Delivery of Service should commence on approximately
