Eight (8) rooms are required, suitable for double occupancy, to be reserved for DVAMC USE ONLY for five (5) nights per week, beginning Sunday of each week and ending Thursday. Contractor shall furnish a compact refrigerator upon request of DVAMC staff, furnish transportation to and from lodging facility and the Lurleen B. Wallace Tumor Institute and the DVAMC. Also, Contractor shall frunish transportation from the lodging facility to and from the DVAMC for evening meals. Contractor must be within a ten (10) mile radius of the DVAMC, 700 So 19th Street, Birmingham, AL. Period of Contract is from 10-1-98 through 9-30-99, with four (4) one year option year renewals. Procurement will be by the source selection process. Offerors will be evaluated on the following factors in descending order of preference and will be requested to provide information to support these factors: 1) Experience and Staffing; 2) Past Performance; 3) Quality of Services; 4) Pricing.
