synopsis/solicitation for Commercial Items prepared in accordance with
the format Subpart 12.6, as supplemented with additional information
included in this notice. This announcement constitutes the only
solicitation; proposals are being requested and a written solicitation
will not be issued. I. The solicitation number is RFP672-170-98. This
is a request for proposal (RFP). II. The solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 97-03. III. This is a 100% Small Business
Set-Aside. The standard industrial classification code is 4953 and the
small business size standard is $6M. IV. 001: Basic Year Collection,
Weekly Pick-Up of Biomedical and/or Pathological waste estimated at
755,000 lbs. per year; Quantity: __ __, Unit: __ __; Unit Price: __
___; Price $__ ___, at VeteransAdministration Medical Center, San Juan,
Puerto Rico. Offer shall include all associated costs for disposal of
biomedical and/or pathological waste. Offerors shall have and submit
with their offer all applicable licenses and permits from the Federal
Government, Commonwealth of Puerto Rico, and the local Environmental
Quality Board ("Junta de Calidad Ambiental"). There are options for
four additional years of services. Offerors shall submit price for base
year and optional years. Base year is from April 6, 1998 to September
30, 1998; Option period I is from October 1, 1998 to September 30,
1999; Option period II is from October 1, 1999 to September 30, 2000;
Option period III is from October 1, 2000 to September 30, 2001; Option
period IV is from October 1, 2001 to September 30, 2002. Provision
52.212-1, Instructions to Offerors -- Commercial Items, is applicable.
The Government will evaluate offers for award purposes by adding the
total price for all options to the total price for the basic
requirement. The Government may determine that an offer is unacceptable
if the option prices are significantly unbalanced. Evaluation of
options shall not obligate the Government to exercise the option(s).
Offeror must submit a completed copy of provision 52.212-3, Offeror
Representations and Certifications -- Commercial Items, with its
proposal. Clause 52.212-4, Contract Terms and Conditions -- Commercial
Items is applicable to this acquisition. Clause 52.212-5, Contract
Terms and Conditions, Required to Implement Statutes or Executive
Orders -- Commercial Items, applies to this acquisition. In addition
Clause 52.217-5, Evaluation of Options, 52.217-9, Option to Extend
Services, 52.217-9, Option to Extend the Term of the Contract, and
52.228-5, Insurance -- Work on a Government Installation, applies to
this acquisition, 52.246-20, Warranty of Services and 852.210-77 also
applies. VII. A Firm Fixed Price Contract will be awarded. Proposals
shall be submitted to the address herein or faxed to 787-766-6035 and
directed to the attention of Rey F. Clemente (90C). The Government
shall not be held responsible for quality of fax transmittals. The
solicitation number (RFP #) shall be listed on the outside of your
