(440) 838-6091 The VA Medical Centers, Cleveland, Ohio, propose to
enter into a contract for the services of an Instructor to teach a
comprehensive 2 week training program for 10-12 students on the Repair
and Maintenance of the Department of Veterans Affairs (VA) Diagnostic
Ultrasound Focusing systems. This is a Request for Quotations (RFQ),
for which it and all incorporated provisions are those in effect
through Federal Acquisition Circular 90-32. This acquisition is
set-aside for small business for which the Standard Industrial Code is
8748 and the small business size standard is $5.0 million. It is
anticipated that a firm fixed price contract will be awarded. This is
a combined synopsis/solicitation for commercial items prepared in
accordance with the format in FAR subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; quotations are being requested and
a written solicitation will not be issued. The Contractor shall provide
the services/supplies identified as follows: 1) Contractor shall
provide equipment specific models listed below in Item 7, for students
to train on. 2) Contractor shall provide student workbooks, lab
manuals, quizzes and tests. 3) The contractor shall issue a final exam
to determine whether students have achieved the capability to
troubleshoot Ultrasound systems within 70% accuracy. 4) Contractor
shall submit final grades to the Contracting Officer's Technical
Representative (COTR) within 72 hours of the final exam. 5) Contractor
shall provide additional training (at no cost to the government) for
students, who are not able to perform within the parameters when he/she
returns to their work site. The parameters are: a) Identify the
characteristics of acoustic waves; b) describe the generation and
propagation of the ultrasound beam; c) describe operation of A-mode,
M-mode, Doppler and 2D imaging; d) follow circuit operation of Acuson
128, ATL Ultramark, and Hewlett Packard systems; e) troubleshoot and
calibrate systems and run diagnostic software. 6) Contractor shall
provide a contingency plan to provide a qualified replacement
Instructor in case of unforeseen unavailability or loss of that
individual from any task. 7) Contractor shall be knowledgeable in the
repair and maintenance of medical ultrasound equipment specifically the
ACUSON 128, Hewlett Packard HP1000, and ATL Ultramark 4 systems with 5
years experience in teaching Medical Ultrasound equipment repair and
maintenance. Training shall be conducted at the Little Rock Employee
Education Center, North Little Rock, AR. Contractor shall be cable of
instructing the classes Monday through Friday from 8:30 AM to 4:30 PM.
Contractor shall follow the established program outline developed by
the Little Rock Education Center. Classroom, Lab Space and lodging will
be provided at no charge to the instructor. Program outline and
materials will be submitted to the COTR to determine if they meet the
intended purpose of the class, before placing into use. The COTR will
approve or disapprove Contractor submittals. All materials
developed/produced by the contractor for this program will become the
property of the Department of Veterans Affairs, including copyrights to
the workbooks, lab manuals, quizzes and tests. Award of a firm fixed
price contract is contemplated. FOB Point is North Little Rock, AR.

EVALUATION -- COMMERCIAL ITEMS (OCT 95) with the following evaluation
criteria: 1)Proposed instructors educational background and technical
knowledge on instruments identified; 2) Instructors past experience; 3)
Instructors ability to meet requirements. The evaluation criteria are
considered equal in importance with price. Sources responding to this
announcement shall provide: a price proposal which includes all cost
including travel, prompt payment terms, remittance address, synopsis
addressing evaluation criteria, three or more references, a completed
copy of FAR 52.212-3 and VAAR 852.219-70. All responsible sources may
