V -- CONSOLIDATED AMBULANCE SERVICE SOL RFP 662-28-98 DUE 033198 POC Daryl Hanson (415) 750-2172 The contractor shall provide twenty-four (24) hour ambulance service for the beneficiaries of the Department of Veterans Affairs. Service shall include all labor, materials, equipment and supplies required to provide both BLS and ALS ambulance service for the VA Medical Center-San Francisco and the VA Northern California Health Care System. The DVA reserves the right to award muliple contracts. The Government shall award a contract(s) resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1. Cost Factor; 2. Adequacy of Facilities/Vehicles; 3. Record of Past Performance; 4. Qualifications and Experience of Personnel; and 5. Technical Approach and Proposed Schedules in line with available resources. Contract shall be for a base year (or portion thereof) and four (4) one year options.
