Officer, Linda Dean, (713) 794-7405 This is a combined
synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. The solicitation is issued as a
request for quotation (RFQ 580-8-LD). The solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 97-03. Line item number 1 -- Contractor shall
provide 1 each mobile health van, custom designed with equipped front
cabin, attic storage, reception area, nurses' station and exam room.
Van shall not exceed 20' long, 8-1/2" wide, and 12-1/2' overall height.
Van must be self-propelled and conform to all ADA requirements. Paint
color(s) and logo to selected after award but must be included in bid.
Finish and color must be premium quality powder coat, baked on epoxy.
As a minimum, van must have the following general basic features: Exam
Room with cabin alarm system with panic buttons; air deflectors on
ceiling registers; diagnostic instruments; professional exam tables;
screened and tinted windows; and swing door. Nurses' Station with
acoustical ceiling height of approx. 8'; medical cabinets which comply
with all Joint Commission standards for sanitation and workmanship
(exact configurations, colors, etc, to be determined by VA from
currently manufactured items); ADA compatible scale; powered chair
lift; and fold-down phlebotomy desk with scope above. Reception Area
with restroom with noise control capabilities; emergency window area;
counter/desk for medical records/patient registration; convenient flip
seating; water station; TV/VCR shelf; microwave shelf; and AM/FM
shelf. Storage Attic with equipment storage space of over 160 cu.ft.
and exterior receptacle wire inside of water box. Must be equipped with
flag mount, pole, flag, and portable folding stairs. Additional
mechanical/engineering requirements: 4-wheel power disc brakes with
backup braking; power steering with intermittent wipers; C-channel 8
class frame design; leak-proof roof system; all around steel belted
radials and disk brakes; water storage tanks with free for shore
adaptability; Cat 3116 diesel engine (gasoline comparable with
automatic transmission engine preferred) with appropriate electric
generator; noise eliminators throughout; storage compartments for
supplies; minimum of 2 years of limited miles warranty on chassis with
certified or licensed professional service/repair locations located
within Houston metropolitan statistical area; computerized leveling
gear; smoke detectors; fire extinguishers; phone/modem jack;
health-care electrical wiring; rear and front walls air-conditioning
mount; all season rib tires, 255/80R x 22.5G; water heater. Quoter
shall submit a conceptional drawing of the vehicle proposed for this
solicitation detailing all the standard items/features for vehicle. A
specific list of optional features and prices should also be included.
Additionally, a minimum of 1000 postcards with reverse printing as
specified by VA shall be provided picturing the completed van for this
facility. Van must be delivered and accepted within 120 days after
receipt of purchase order and delivered FOB: VA Medical Center,
Houston, Texas. FAR provision 52.212-1, Instructions to Offerors --
Commercial, applies to this acquisition. The Government will award a
contract resulting from this solicitation to the responsible offeror
whose offer conforming to the solicitation provides the best value to
the Government. Offerors must include completed copy of FAR provision
52.212-3, Offeror Representations and Certifications -- Commercial
Items, with its offer. FAR clause 52.212-4, Contract Terms and
Conditions, applies to this acquisition. The following clauses are
addenda to this clause: 52.232.33, Mandatory Information for Electronic
Funds Transfer Payment, and 852.270-4, Commercial Advertising. FAR
clause 52.212-5, Contract Terms and Conditions Required to Implement
Statutes or Executive Orders -- Commercial Items, and FAR clauses
52.203-6, 52.203-10, 52.219-8, 52.222-26, 52.222-35, 52.222-36,
52.222-37, and 52.225-3 also apply to this solicitation. Contractor
must guarantee van, all equipment, accessories, etc., against defective
material, workmanship and performance for a minimum period of 1 year.
Maintenance manuals, handbooks, instructions, etc., must be provided
for all equipment as well as the vehicle. Offers must be submitted by
3:00 p.m., March 18, 1998, to Contracting Office (90C), VA Medical
