The VA Medical Center, Huntington, WV intends to issue a solicitation for Community Based Primary Care Services. Contractor is to provide Primary Care services to veterans of the Department of Veterans Affairs Medical Center, Huntingotn, WV for the period of one year from the date of Notice to Proceed. Veterans will be referred to the contractor for Primary Care Services from the Huntington, VA Medical Center. Veterans to be serviced under this capitation contract reside primarily in Charleston, WV and surrounding counties. Contractor's facility to provide service, must physically be located within the city limits of Chalreston, WV or no more than (5) miles outside the corporate city limits of Charleston, WV. This contract will include Base Year with two one year renewal options, that can be exercised at the option of the Government. Contractor's facility must meet the standards set forth 42 code of Federal Regulations, Chapter 4, Part 49, Subpart A Rural Health Clinics: for certification and applicable and current JCAHO standards.
