synopsis/solicitation for the commercial items prepared in accordance
with the format in FAR subpart 12.6 as supplemented with the additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. This solicitation, No. F02604-98-Q0197
is issued as a Request For Quotation (RFQ). This solicitation document
and incorporated provisions and clauses are those in effect through
Federal Acquisition Circular 97-2. This acquisition is set-aside for
100% small business concerns; SIC: 1795; Size Std: $7 million dollars.
All responsible sources may submit a quotation which shall be
considered. MINIMUM NEEDS OF THE GOVERNMENT: INSTALLATION AND LOCATION:
Luke Air Force Base (AFB), Arizona. The site is located in the
southwestern area of the base adjacent to the South Arm/Dearm apron.
Use South Gate and Super Sabre Street for site access. The Civil
Engineering Squadron controls access to the site and a gate key must be
obtained from them for entry. All work to include trucking of materials
off Luke AFB property shall be accomplished during the week Wednesday
thru Friday 0730 to 1630 hours. The Environmental Flight Compliance
Section Chief, Mr Jeff Rothrock, will serve as the AF Project Manager
and will be responsible for the acceptance of all work performed under
this contract. SUMMARY STATEMENT OF WORK: The Contractor shall furnish
all equipment, labor, and materials necessary to remove, relocate,
recycle, transport and dispose of approximately 200 tons of
construction debris and 2,300 tons of asphalt and concrete. Contractor
shall recycle all asphalt, concrete, metal, and other debris as much
as possible. Screen material to the size of 4" minus and stockpile
residue on site. DESCRIPTION OF WORK: The Contractor shall perform the
following tasks to complete this statement of work: Task 1. The
contractor is required to: (1) Obtain and submit prior to site work a
Maricopa County Dust Control Permit; (2) Obtain and submit prior to
site work a Civil Engineering Dig Permit; (3) Submitt prior to site
work a Health & Safety Plan emphasizing caution to be excercised when
aircraft are on the Arm/Dearm apron; (4) The Contractor shall name and
assign a responsible person as Project Manager. The Project Manager
shall maintain the project file to contain all correspondence and
documents pertinent to this project and shall provide 56 CES/CEVC with
the names of the individuals responsible for preparation and
coordination of the project work; (5) The Project Manager, working as
team leader, will be responsible for project coordination and charged
with overall project responsibilities. Task 2. The contractor is
required to perform the the following site work: (1) The Contractor
shall furnish all equipment, labor, and materials necessary to remove,
relocate, recycle, transport and dispose of approximately 200 tons of
construction debris and 2,300 tons of asphalt and concrete. Contractor
shall recycle all asphalt, concrete, metal, and other debris as much as
possible; (2) Screen the debris material to the size of 4" minus and
stock-pile residue on site per the direction of the Air Force project
manager. (3) Level the site; (4) Work must be accomplished Wednesday
thru Friday 0730-1630 hours so as to avoid high aircraft traffic days
typical of Monday thru Tuesday. Task 3 The contractor shall report the
tonage of recycled material by category. SCHEDULE AND SUBMITTALS The
contractor shall develop a schedule which incorporates all critical
work elements, submittals and review periods. ITEMS AND DATA TO BE
FURNISHED BY THE GOVERNMENT The AF Project Manager or his designee will
coordinate site access and site layout to include any job trailer
placement or equipment storage requirements. The contractor is
responsible for arranging all required utilities. DEVIATION FROM SCOPE
OF WORK The contractor shall not depart from nor perform work outside
of this Statement of Work and criteria on which the Statement of Work
is based without the written direction of the Contracting Officer
(CO). If the contractor performs such work without written approval, it
does so at its own risk. Any changes to this SOW will be coordinated
with 56 CES/CEVC as well as the CO. VISIT TO SITE, PRIVATE SOURCES, AND
GOVERNMENTAL AGENCIES The contractor shall advise the AF Project
Manager of each proposed visit to the site, private sources, and
government agencies (federal, state, county and city) prior to each
visit. Desired Completion Date 120 days after receipt of award. FOB:
Luke AFB, AZ. The provisions of FAR 52.212-1, INSTRUCTIONS TO OFFERORS
-- COMMERCIAL ITEMS, Reference FAR 52.212-1 para (c) is hereby
tailored as follows: (a) Period for acceptance of offers. The offeror
agrees to hold the prices in its offer firm for 60 calendar days from
the date specified for the receipt of offers. Reference FAR 52.212-1
para (b) is hereby tailored as follows: (b) Submission of offers. A.
Purpose: These instructions are designed to ensure the submission of
information to provide for the understanding and comprehensive
validation of the proposal. Offerors are cautioned to follow the
detailed instructions fully and carefully, as the government reserves
the right to make an award based on initial offers received without
