COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN
ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH
ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT
CONSTITUTES THE ONLY SOLICITATION; BIDS ARE BEING REQUESTED AND A
WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation is issued as
Request for Proposal No. RP98-005 (Submission No. 152196), Amendment
003. This solicitation document and incorporated provisions and clauses
are those in effect through Federal Acquisition Circular 90-46. The
specifications provided herein are substituted in lieu of those stated
in the notice posted 12/18/97. The successful offeror shall furnish
and install at the National Institutes for Occupational Safety and
Health (NIOSH), Morgantown, WV one (1) high performance liquid
chromatograph (HPLC) system with components Brand Name or Equal to the
following items manufactured by GBC Separations, Concord, MA. Contract
Line Item (CLIN) #1, Model LC1150 (Part 100-1155), Multisolvent
Quaternaray Solvent Delivery System with in-line membrane degasser and
system organizer; CLIN #2, Model LC5000 (Part 99-1234-00) Photodiode
Array Detector, with Win Solo; CLIN #3, Model LC1250 (Part 100- 1250),
Fluorescent Detector; CLIN #4, Model 1650 (Part 99-0269-51), Advanced
Auto-sampler with 160 x 2 ML vial tray; CLIN #5, WinChrom v1.3 (Part
99-0000- 00), Windows based Chromatography Data Management System (2-2)
with PDA control and display; CLIN #6, Temperature Control Module,
0.01C precision; CLIN #7, Model FC203B (Part 171011DV) Automated
Fraction Collector with 3- way valve, to include valve kit collection
plate, code 1 racks, pan tube bar and inlet tubing (manufactured by

Delivery is desired no later than 30 days after receipt of contract.
EVALUATION FACTORS: Offers will be evaluated on their ability to meet
or exceed the stated requirements and demonstrated past performance on
contracts of similar size and complexity to this requirement (include
names and addresses of Company/ Government Agency, name and telephone
numbers of customer contacts along with contract number, and
description of work performed with your offer.) Technical proposals and
cost or price are approximately equal in importance. The Government
intends to make a single award to the responsible offeror whose
proposal conforms to all requirements of the solicitation and is
considered the best value to the Government. Offers shall be proposed
based on F.O.B. Destination (Morgantown, West Virginia) and the
requirements at FAR 52.214.21 "Descriptive Literature" apply. The FAR
provisions and address for receipt of offers remain as stated in the
original CBD notice posted 12/18/97. Offerors shall ensure that the
required forms, price breakdown of components, descriptive literature
and a delivery schedule are provided with their offer. This AMENDMENT
003 MUST BE ACKNOWLEDGED with your offer. Facsimile offers are not
authorized. Offers that fail to furnish the required information, or
reject the terms and conditions of the solicitation may be excluded
