Wayne, Indianapolis and Marion, Indiana; and Danville, Illinois, also know as VISN 11. Contractor will provide all supplies, materials, equipment, labor, supervision, management, transportation, patient education, safety management, infection control, develop mechanisms and methods to register and respond to complaints, issue approvaed Patient Rights and Responsibilities and provide all pertinent documentation.
