combined/synopsis under DAAC89-98-Q-0001 advertised on November 26,
1997 issue, is amended as follows: The solicitation/quotation number is
hereby corrected to read, DAAC89-98-T-0025, any offers received prior
to this notice change will still be considered unless otherwise
notified by the offeror. This will be issued/awarded as a Firm Fixed
Price/Time and Material Contract for services. The government will
reimburse the contractor for labor and materials by issuance of any
task order, authorizing the contractor to repair such item. The
contractor shall only include all cost associated with the preventitive
maintenance requirements. Any cost associated with the repair/breakdown
maintenance will be negotiated and authorized by issuing a task order
under this contract. Since the government does not have the manuals for
the equipment listed, any manuals that the contractor obtains, can be
reimbursed by the government, provided a proper invoice of cost is
submitted and after receipt of payment the manual shall become the
property and remain the property of the government. The award of this
quotation is "Subject to Availability of Funds". The contractor shall
indicate in his offer, the acceptance period of prices offered. The
government's minimum acceptable period is 30 days from Closing date for
these offers. The contractor shall include a complete copy of Clause
52.212-3, not Clause 52.232-3 which was referenced in previous synopsis
and is hereby deleted. Clauses 52.21204 and 52.21205 referenced in
previous synopsis is corrected to read 52.212-4 and 512.212-5. In
addition to previous referenced clauses, the following clauses are
hereby being added and shall be complied with: 52.232-7, 52.232-33,
52.232-3, 52.245-5, 52.217-5, 52.217-9, 52.222-42, 52.222-43 and
52.228-5. Upon request, this office will provide any referenced clause
above, in full text. Clins 0001, The contractor shall provide labor,
materials and equipment necessary for preventative maintenance
requirements as specified, on all equipment listed under this contract.
The contractor shall provide a monthly cost for a one year period (12
months) and separate monthly cost for a year period for each option (
3 years to be included). The period of this contract, if options are
exercised, shall be for a total of 4 years). Clins 0002, Labor and
material for any necessary repairs/breakdowns maintenance authorized by
issuance of a task order which will identify the labor and material
cost payable to the contractor for the necessary repair. Labor hours
shall be specified fixed hourly rates that includes wages, overhead,
G&A expenses, and profit. Material costs shall be paid at actual
purchase costs. Pricing of material shall be in accordance with FAR
52.232-7, paragraph (b)(3). A ceiling price of $65,800 is applied to
this line item. Unless written approval is obtained from the
contracting officer, this ceiling shall not be exceeded. If exceeded
without written approval shall be at the contractor own risk. The
contractor shall provide a price for the hourly rate used for repairs
issued under individual task orders. Include an hourly rate for the
base year and each option year, if exercised. For evaluation purposes
only to determine the lowest price offered from a responsible offeror,
the government will estimate a maximum of 1000 labor hours of effort
for repairs for each year. The date specified for receipt of offers is
being extended to 23 December 1997. All offers/amended offers shall be
received in this office by COB (1700 hours). Any quote that has already
been received will be considered to comply with the amended changes if
no other notification is received by the contractor by the revised
closing date. All other terms and conditions previously submitted
