The VA North Texas Health Care System, 4500 S. Lancaster Road, Dallas, TX 75216 has a requirement for one (1) Base Outpatient Clinic in each of the following counties: Hunt, Hopkins, Red River, Rockwall, Kaufman, Franklin, Titus. The Contractor shall provide for the delivery and management of continuous primary and preventive medical care for enrolled VA patients. The contractor shall provide health care providers, medical facilities, medical equipment and supplies, pharmaceuticals, and all administration functions to fulfill all levels and ranges of care expected for an initial one year period and four (4) subsequent one year renewals. Compensation will be made through a monthly capitation payment (per member per month). Issue date will be on, or about 1/9/98. Proposal due date will be on, or about 2/6/98.
