Lancaster Road, Dallas, TX 75216, has a requirement for one (1) Community Based Outpatient Clinic in each of the following counties: The Texas counties of Delta, Lamar and Grayson and the Oklahoma counties of Bryan and Choctaw. The contractor shall provide for the delivery and management of continuous primary and preventive medical care for enrolled VA patients. The contractor shall provide health care providers, medical facilities, medical equipment and supplies, pharmaceuticals, and all administration functions in accordance with the terms and conditions of the proposed contract. Compensation will be made through a monthly capitation payment. Issue date will be on, or about, 12/12/97. Proposal due date will be on, or about, 1/12/98.
