synopsis/solicitation for commercial items/services prepared in
accordance with format in Sub parts 12/6 and 13.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation to be issued, proposals are being
requested from this announcement, a written solicitation will not be
issued. The Request for Proposal number is 575-438-98. This
solicitation incorporates provisions and clauses in effect through FAC
97-02. Standard Industrialization Code (SIC) of 8999 is applied. This
solicitation is issued subject to the availability of government funds
at time of award. This solicitation is unrestricted for the following
service: Contractor shall provide maintenance services for the
following General Electric X-ray equipment: AMX4, STENO/D6-D9,
SFX30/MVP, RAD MVP60, SFX30/ADVTX. Maintenance is to include two (2)
preventive maintenance visits per year, all parts & labor during
business hours (8-5 Monday thru Friday), on phone service, on line
service through the modem, and manufacturer service software to be
included (bring in a lap top computer to hook up to the system to
diagnose and service the system). The following provisions and clauses
are applicable and are hereby incorporated into the solicitation by
reference and will be provided by the contracting officer in full text
upon request: FAR 52.212-1, Instructions to Offerors Commercial Item
applies. Addendum clause FAR 52.215-18, Facsimile Proposals also
applies. Offerors are required to complete and include a copy of the
following provisions with their proposals: FAR 52.212-3, Offeror
Representations and Certifications Commercial Items, FAR 52.212-5,
Affirmative Action for Special Disabled and Vietnam Era Veterans; FAR
52.222-36, Affirmative Action for Handicapped Workers; FAR 52.222-37,
Employment Reports on Special Disabled Veterans and Veterans of the
Vietnam Era. Award will be made to that offeror (1) whose proposal is
technically acceptable and (2) whose technical/cost relationship is the
most advantageous to the Government. While cost is secondary to
technical, it will be a factor in the award decision. Also, award may
not necessarily be made for technical capabilities that would appear to
exceed those needed for the successful performance of the work.
